大肠埃希菌对头孢他啶耐药的危险因素分析 被引量:2

摘要 目的:探索产超广谱β-内酰胺酶(ESBLs)大肠埃希菌与非产ESBLs菌株对头孢他定耐药的危险因素。方法:以2004~2005年我院分离出的186株大肠埃希菌为研究对象。对患者病历做回顾性调查。用SPSS10.0进行统计学分析★检验比较各组间率的差异。在单因素分析的基础上,以是否耐药为因变量,用Logistic逐步回归分析有关因素与细菌耐药的关系。结果:单因素分析表明,产ESBLs大肠埃希菌与非产ESBL菌株对头孢他啶(CAZ)耐药的危险因素不尽相同,包括既往病史、现患疾病及状态、住院时间、混合感染、侵袭性操作、抗菌药及其他药物的使用及年龄等。而多因素分析显示,产ESBLs大肠埃希菌与非产ESBLs菌株对CAZ耐药的共同因素为年龄≥60岁,住院时间≥3周。结论:高龄、住院时间长及混合感染是我院产ESBLs大肠埃希菌耐药的主要危险因素。 Objective: To analyze the risk factors caused by resistant strains of Escherichia coli to ceftazidime in Hospital. Method: One hundred and eighty-six strains resistant bacteria of Escherichia coli were isolated at our hospital between 2004 - 2005. Statistical analysis was made by SPSS10.0, odds ratios and stepwise Logistic regression analysis were used to determine the interrelationships of drug resistance between multiple variables. Result: The single factor analysis showed that, the risk factor of produced ESBLs Escherichia Coli and non-produced ESBLs Escherichia coli were not same such as history disease, present disease, hospitalization time, mixed infection,invasive operation, antibiotics combined other drugs usage and age. But the mutiple factors anelysis showed that the common factors were age 〉160 years and the hospitalization time ≥3 weeks and mixed in factors. Conclusion: Agepatient, long hospitalization and mixed infection are the main risk factors of drug resistance of E. Coli.
