不同固定对兔胫骨骨折愈合的影响 被引量:1

Empirical observation on the influence of different fixations on union of fracture in rabbit's tibia

摘要 目的:观察不同固定对实验性兔胫骨骨折愈合的影响。方法:选择健康新西兰家兔18只,随机分成A、B、C3组,各组6只。在左胫骨中下1/3处造成3 mm横形骨折模型。A、B两组用石膏固定d天后分别换成小夹板外固定,将压力传感器置于前侧小夹板内面骨折断端处,A、B两组夹板上扎带的松紧度不同,A、B组扎带上下移动分别约为3 mm和7 mm,此时,读出A组压力传感器读数是18 kPa;B组压力传感器读数是12 kPa。C组用四孔钢板内固定。分别于术前和造模固定后2、4、6周进行血清碱性磷酸酶(ALP)测定,对其数据进行统计学分析。于第6周处死动物,对骨痂生物力学进行测试。结果:A组的抗拉强度显著高于B组和C组。术后2周,三组动物ALP均显著增高,以A组最为显著。术后4周,B组和C组ALP持续升高,A组开始下降,但仍高于术前;C组与B组变化不明显。术后6周,A组ALP已降至正常,B组和C组仍不正常。结论:恰当的小夹板固定能促进骨折断端矿物质的沉积,提高骨折愈合质量,缩短骨折愈合时间。 Objective : To observe the influence of different fixations on union of fracture in rabbits tibia for the experiment. Methods:I8 cases of healthy Newzealand rabbits were choosed and randomly divided into three groups ,group A ,group B and group C ,6 eases in each group. Models were built by 3mm transverse fracture made on the place of middle and below 1/3 in left tibia. Animals of groups A and B were fixed by cypsum fibrosum for 5 days, then changed for external fixation with small splint, at the same time the baroceptor were placed on the broken ends of fractured bone in ventri-side of small splint,and the degree of prick tied in two groups were different. The distance of them wagged were respectively 3 mm and 7 mm. Then the count of baroceptor in group A was 18 kPa,that of group B was I2 kPa. Four-hole steel plates were used for internal fixation in group C ,the animals were detected with ALP on points of pre-operaion and 2 weeks,4 weeks and 6 weeks post model-building and fixation. The datas above were made for statistical analysis. On time of 6 weeks animals of all groups were killed and vitodynamics on bony callus of them were measured. Results: The tensile strength of group A was super to those 9f groups B and C. 2 weeks post-operation, the volumes of ALP in three groups all notably increased, the volumes of group A was the most. 4 weeks post- operation, the volumes above in groups of B and C kept on increasing, but that of group A began to lower and was higher than that pre-operation. There had no obvious changes in groups of B and C. 6 weeks post-operation, the volume of group A dropped to the normal, but those of group B and group C still remained abnormal. Conclusion: The correct fixation with small splint could promote deposition of mineral matter in broken ends of fractured bone, elevate the quality on union of fracture and shorten times on union of fracture.
