强化胰岛素治疗对脑出血伴糖尿病患者的预后影响 被引量:2

摘要 目的观察强化胰岛素治疗对高血压脑出血伴糖尿病患者预后的影响。方法将有糖尿病史的高血压脑出血患者62例和高血压脑出血伴新诊断的糖尿病患者43例随机分为强化胰岛素治疗组(53例)和对照组(52例),经过3~6个月随访。根据GOS预后评分观察强化胰岛素治疗组与对照组的预后。结果比较2组预后,强化胰岛素治疗组恢复良好的患者增加,致残率下降,病死率降低。2组比较差异均有统计学意义(P<0.01)。结论高血糖加重高血压脑出血后脑损害,使其预后不良,应用强化胰岛素治疗可明显改变伴有糖尿病脑出血患者的预后。 Objective To observe the therapeutic effect of intensive insulin treatment on the prognosis of the diabetic patients complicated with hypertensive intracerebrial hemorrhage. Methods 105 patients were divided into two groups : control group and intensive insulin treatment group. After 3 to 6 months follow up, the patients prognoses were analyzed by GOS scoring. Results As compared with control group, insulin treatment group had a better recovery, and the mortality in intensive insulin treatment group was decreased. There was a significant difference between two groups ( P 〈 0. 01 ). Conclusion Hyperglycemia may aggravate the brain damage after hypertensive intracerebrial hemorrhage, resulting in worse prognosis, however, intensive insulin treatment can improve obviously the prognosis of diabetic patients complicated with hypertensive intracerebrial hemorrhage.
