摘要
【目的】观察高血压病(essential hypertension,EH)合并2型糖尿病患者(type2 diabetes mellitus,T2DM)的心率震荡(heart rate turbulence,HRT)和心率变异(heart rate variability,HRV)的变化。【方法】EH合并T2DM者84例、EH80例,健康对照组60例,记录24h动态心电图,计算HRT的初始值(TO)和震荡斜率值(TS)以及SDNN、PNN50和LF/HF,比较三组之间的差别,分析HRT和HRV之间的相关性。【结果】EH合并T2DM和EH患者的HRT和HRV较正常对照组均有明显异常(P〈0.05),EH合并T2DM患者的TS和LF/HF较EH组明显降低(P〈0.05);HRV的各项指标和HRT均有良好的相关性,其中PNN50与HRT相关系数最高。【结论】EH患者有显著的HRT减弱及HRV的异常,其中EH合并T2DM较单纯的EH患者更加明显,且HRT与HRV的密切相关,再次证明心脏自主神经功能异常在EH,特别是EH并T2DM中的重要作用。
[Objective] To observe the changes of heart rate turbulence (HRT) and heart rate variability (HRV) in patients with essential hypertension(EH) and type 2 diabetes mellitus(T2DM) with EH . [Methods] The total of 84 patients with T2DM with EH, 80 patients with EH and 58 control subjects were investigated . Turbulence onset(TO), turbulence slope(TS) , SDNN, PNN50 and LF/ HF were measured by using recorded 24 hours dynamic electrocardiogram. The differences among them were compared, and the correlation between them was analyzed. [Results] All of the values of HRT and HRV were significantly different between T2DM with EH or EH group and control group ( P〈0.05). TS and LF/ HF were significantly lower in T2DM with EH group than those of EH group ( P〈0. 05). Good correlations were found between HRT and HRV parameters. The correlation coefficient between PNN50 and TO/TS were the highest. [Conclusion] HRT and HRV in patients with EH are significantly abnormal and they are more significant in patients with T2DM with EH than in patients with EH. And there is a strong correlation between HRT and HRV. It suggests that cardiac autonomic nerve dysfunction perhaps plays an important role in EH, especially in T2DM with EH.
