摘要
目的：研究并分析采用AO微型钢板螺钉内固定治疗手部骨折的技术及其疗效。方法：对采用手术治疗并获得随访的48例（52处）手部掌、指骨骨折患者的临床资料进行分析。采用切开复位,AO微型钢板螺钉内固定术,术后进行早期功能锻炼。结果：本组病例随访时间6-12个月,平均6.8个月。48例患者术后创口均Ⅰ期愈合,无伤口感染及骨髓炎发生。所有病例骨折均愈合,临床愈合时间5-7周,骨性愈合时间8-10周。所有病例均未发生术后骨折移位或畸形愈合。按照手指总主动活动度（total active motion,TAM）的评级标准并参考手指总主动屈曲度（total active flexion,TAF）的评分标准进行评定,优32例（34处）,良13例（15处）,差3例（3处）,优良率为93.7%。结论：切开复位、AO微型钢板螺钉内固定术是治疗手部掌、指骨斜形或螺旋形骨折及关节内骨折的一种有效方法。选择适宜的手术适应证,掌握熟练的手术技巧,及时进行早期功能锻炼是获得满意疗效的关键。
Objective： to investigate and analyze the treatment outcome and technique of internal fixation with AOmini - platescrew for treatment of hand fractures. Methods： 48patients （52 lesions） in our hospital suffured from hand fracture were studied retrospectively The fractures of all cases were treated operatively with open reduction and internal fixation with AO mini -platescrew, and the early rehabilitation were performed after operation the evaluation of total active motion （TAM） and total active flexion （TAF） were used. Results： The average duration of the follow -up was 6. 8 month （range from 6 to 12 months）. Of these 48 cases, the incisions were all healed by first intention. All fractures union were obtained without malunin and deleyed union. The healing time was 5 -7 weeks, the results excellent cases were 32 （34 lesions） , good were 13 （15 lesions）, overall excellent and good rate were 93.7%. Conclusion： Open reduction and internal fixation with AO mini - platescrew is an effective way for treatment of the oblique shape or spiral fracture of metacarpal and phalanx and the intra - articular avulsion fracture, suitable indication, skillful operation, timely early exercises are the key ways of obtaining satisfactory treatment outcome.
