摘要
目的:探讨瘢痕子宫中孕引产的方法及安全性。方法:选择我院2007年1月～2008年12月47例瘢痕子宫中孕引产病例作为研究组,随机抽取60例非瘢痕子宫中孕引产病例作为对照组;孕13～14周者行米非司酮配伍米索前列醇引产,其中,瘢痕子宫24例,非瘢痕子宫32例;孕15～27周者行羊膜腔内注射利凡诺引产术,其中,瘢痕子宫23例,非瘢痕子宫28例。分别观察两组用药后宫缩开始时间、总产程、产后2h出血量、并发症、住院时间等指标,并进行比较。结果:孕13～14周采用米非司酮配伍米索前列醇引产及15～27周采用羊膜腔内注射利凡诺引产术,研究组与对照组比较,年龄、孕周、用药至宫缩开始时间、总产程、产后2h出血量、并发症及住院时间比较,无显著性差异(P>0.05)。结论:瘢痕子宫与非瘢痕子宫中期妊娠采用米非司酮配伍米索前列醇引产及羊膜腔内注射利凡诺引产无显著性差异,这两种引产方法均安全、有效、价廉。
Objective： To investigate the methods and security used for inducing abortion with scar uterus of midtrimester pregnancy. Methods： 47 cases with scar uterus requesting abortion in midpregnaney from January 2007 to December 2008 were selected as study group. 60 cases with no-scar uterus requesting abortion in midpregnancy were randomly selected as control group. Women who were pregnant between 13 and 14 weeks were given Mifepristone combined with Misoprostol, in these women, 24 cases were scar uterus and 32 cases were no-scar uterus. Women who were pregnant between 15 and 27 weeks were given Rivanol injected into the amniotic cavity, in these women, 23 cases were scar uterus and 28 cases were no-scar. Then the targets of the start time of uterine contraction after medication, the total time of abortion, the amount of bleeding 2 h after placenta expulsion, the complications and the time be in hospital were observed and analyzed. Results： There was no significant difference between study group and control group in age, gestational weeks, the targets of the start time of uterine contraction after medication, the total time of abortion, the amount of bleeding 2 h after placenta expulsion, the complications and the time be in hospital （P〉0.05）. Conclusion： In abortion of midtrimester pregnancy, pregnancy with scar uterus has no significant difference from pregnancy with no-scar uterus. It is save, efficient and cheap for inducing abortion with scar uterus of midtrimester pregnancy to use Mifepristone combined with Misoprostol and Rivanol injected into the amniotic cavity.
