摘要
目的：分析Ｃｏｔｒｅｌ－Ｄｕｂｏｕｓｓｅｔ器械治疗脊柱侧凸过程中出现的各种并发症，并提出预防及治疗措施。方法：１９９３年２月～１９９６年９月共治疗各种原因所致的脊柱侧凸３８例，平均年龄１６．４岁，术前Ｃｏｂｂ角为４０°～１３０°，平均６８°。结果：术后平均矫正率６１％。术中及术后共发生各种并发症９例，其中椎板、椎弓骨折４例，Ｃ－Ｄ棒末端过长２例，神经损伤１例，术后肺功能衰竭１例，螺钉遗留１例。结论：Ｃ－Ｄ器械治疗脊柱侧凸的效果好。
bjective: To analyse the causes of the complications and discuss how to prevent and manage them during correction of scoliosis using Cotrel- Dubousset instrumentation. Methods: Thirty eight cases of scoliosis were corrected by Cotrel- Dubousset instrumentation at an average age of 16.4 years from February 1993 to September 1996. The average preoperative Cobb angle was 68° (range 40° - 130°). Results: The average rate of correction was 61% , meanwhile complications were found in 9 cases. There were intraoperative laminar or pedicle fractures in 4, bulge of C- D rods in 2, neurological injury in 1, postoperative pulmonary failure in 1 and screw left over in 1. Conclusion: Attention shoud be paid to all kinds of complications although Cotrel- Dubousset instrumentation is an excellent tool for the correction of scoliosis.
