摘要
目的 探讨放疗前食管X片凶险征是否为独立预后因子及有该征者治疗失败表型.方法 104例患者入组,其中放疗前X片有凶险征者28例,无者76例,病人均采用单纯放疗.结果 有X片凶险征者3年生存率14%,3年局控率为25%显著低于无X片凶险征者的40%,41%(P值分别0.006,0.032);而远处转移率为27%显著高于无者的18%(P=0.048).经Cox多因素分析,有X片凶险征在生存,局控及远处转移等三项指标中均有统计学差异,P值分别为0.004,0.01及0.01.结论X片凶险征为提示中胸段食管癌预后的独立因子.有X片凶险征者治疗失败不仅在局部复发而且有全身转移.
PURPOSE To evaluate whether an unfavourable radiological sign was an independent predictor of failure patterns for patients with middle esophageal carcinoma. METHODS A total of 104 patients with middle esophageal carcinoma entered into this study. Twenty-eight patients showed unfavourable radiological signs and 76 patients did not. All patients were treated by radiotherapy alone. RESULTS 1) The 3 year actuarial survival, local control rate were 14% and 25% respectively in patients with poor X-film sign, which were worse than that of cases without poor X-film sign (P= 0. 006 and 0. 032) . Distant metastasis developed more in cases with poor X-film sign than those without poor X-film sign at 3rd year (27% vs 18%, P = 0. 048). 2) Cox analysis showed that unfavourable radiological sign was an independent predictor for survival (P = 0. 004). local control (P = 0. 01) and distant metastasis (P = 0. 01) . CONCLUSION 1) Unfavourable radiological sign was an independent prognosticator. 2) The causes of treatment failure in patients with poor X-film sign were not only local but also distant metastasis.
