摘要
目的:观察中医健脾补肾祛瘀解毒法对含5-FU/CF的FOLFOX4化疗方案的减毒增效作用。方法:本组病例仅为小样本叙述性研究(Ddscriptive Study),26例晚期结直肠癌患者均采用FOLFOX4方案化疗,化疗期间和化疗后2周内配合健脾补肾祛瘀解毒中药,观察近期疗效、毒副反应、生存质量变化。结果:近期疗效中CR2例,PR7例,SD13例,PD5例,总有效率(CR+PR)8例,占34.6%,受益率(CR+PR+SD)21例,占80.7%;毒副反应以恶心、呕吐、腹泻为主,多为I^II度,骨髓抑制为I^II度,患者耐受性良好;生存质量上治疗前卡氏评分为(79.625±7.200),治疗后卡氏评分为(73.461±9.356),治疗前后比较卡氏评分有所下降,但卡氏评分降低<10分,因此生存质量稳定。结论:采用健脾补肾化瘀解毒法配合5-FU/CF类方案化疗治疗中晚期结直肠癌,可能能扩大该类化疗方案的适应范围,让部分因年老体弱,脏器储备功能差而难以耐受化疗的患者重新获得治疗机会,并可减毒增效,提高患者生存质量。
Objective ：To Observe the method of invigorating the spleen and reinforcing the kidney, removing the blood stasis and detoxifying the poison in Traditional Chinese Medicine in reducing toxicity and enhancing effects of FOLFOX4 chemotherapy regimen containing 5 - FU/CFMethods ： This is a small-sample descriptive study. 26 sanlples of advanced stage colorectal carcinoma patients were treated using FOLFOX4 chemotherapy regimen. Herbs prescribed complying with the method of invigorating the spleen and reinforcing the kidney, removing the blood stasis and detoxifying the poison were applied during chemotherapy and for 2 weeks after completion of chemotherapy. Entities such as recent treatment effects, toxic side-effects and changes in quality of life were observed. Results：of recent treatment effects were 2 samples of CR,7 samples of PR,13 samples of SD,5 samples of PD,8 samples of CR ＋ PR,which constituted 34. 6% ;21 samples of CR ＋ PR ＋ SD,which constituted 80. 7%. Toxic side-effects mainly included nausea,vomiting and diarrhea, most of which were Ⅰ -Ⅱdegree. Bone marrow suppression effects were mosdyⅠ -Ⅱ degree. In general patients had good tolerance. In assessment of patients＇ quality of life, Karnofsky score was （79. 625±7.200）before treatments, and （73.461±9. 356） after treatments. Karnofsky score after treatments decreased as compared to that before treatments. Since the decrease in Karnofsky score was 〈 10, the quality of life was considered stable. Conclusion：Using the method of invigorating the spleen and reinforcing the kidney, removing the blood stasis and detoxifying the poison in conjunction with chemotherapy containing 5 - FU/CF to treat advanced-stage colorectal carcinoma may broaden the scope of treatment using such kind of cheinothcrapy regimen. This may enable patients who are originally unable to tolerate chemotherapy due to old age,infirmities and/or weak organ reserve functions to regain treatment opportunities ,to reduce toxicity and to enhance treatment effects,as well as to elevate patients＇ quality of life.
