人工晶体光学部边缘设计预防后囊膜混浊的Meta分析 被引量:3

摘要 目的:系统评价不同光学部边缘设计人工晶体(IOL)对后囊膜混浊(PCO)的预防作用。方法:电子检索Cochrane图书馆、Medline数据库、Embase数据库和中国生物医学文献数据库,获取有关不同IOL预防PCO的相关文献。按照选择标准选取合适的随机对照临床研究进行Meta分析。以PCO发生率和Nd∶YAG激光囊膜切开率为临床治疗终点,以危险差作为治疗效应量,根据异质性检验选择固定效应模型或随机效应模型进行合并分析。结果:总计有10项试验纳入Meta分析,1项植入聚甲基丙烯酸甲酯(PMMA)IOL、3项植入丙烯酸IOL、7项植入硅凝胶IOL。锐利边缘和光滑边缘比较,激光囊膜切开率的合并危险差,PMMA IOL为-25%(-45%,-5%)、丙烯酸IOL为-15%(-22%,-8%)、硅凝胶IOL为-6%(-11%,-1%);后囊膜混浊率的合并危险差,丙烯酸IOL为-32%(-51%,-13%)、硅凝胶IOL为-29%(-36%,-22%)。结论:锐利边缘设计作为产生机械屏障作用的最佳几何构型,具有阻挡晶状体上皮细胞向视轴生长的屏障效应,可以有效预防PCO,并显著降低激光囊膜切开率。
