摘要
应用CDFI检查35例四肢软组织肿块(简称STME),其中良性肿块25例,恶性肿块10例。结果显示:①以肿块血流信号丰富程度分级,恶性肿块属0、1级占20%(2/10)(良性肿块占62.5%,10/16),属2、3级占80%(8/10)(良性肿块占37.5%,6/16)。②恶性肿块组动脉峰值流速Vs=0.77+0.23m/s,显著高于良性肿块组(0.31±0.12m/s)(P值<0.01)。③恶性肿块组动脉阻力指数RI=81.12%±6.00%,也显著高于良性肿块组(65.50%±12.90%)(P值<0.01)。以Vs≥0.5m/s或/和RI≥75%作为良恶性肿块判断标准,则两维超声(2D)结合CDFI提高了对STME良恶性肿块判断的敏感性、特异性和准确性,其诊断准确性由单纯2D的61.5%(16/26)提高到92.3%(24/26)和80.8%(21/26)(P值<0.05)。
Thirtyfive patients with soft tissue masses in extremity (STME) were studied by CDFI, including 10 malignant and 25 benign masses. The results showed: ①According to the grades of blood flow signal in masses: 20% (2/10) malignant and 62.5% (10/16) benign masses belonged to 0 and 1 grade, 80% (8/10) malignant and 37.5%(6/16) benign masses belonged to 2 and 3 grade. ②The peak flow velocity (Vs) in malignant masses was higher (0.77±0.23 m/s) than those in benign masses (0.31±0.12m/s) (P<0.01). ③The resistance index (RI) in malignant was higher (81.12±6.00%) than that in benign masses (65.50±12.90%)(P<0.01). Combining 2D with CDFI, with Vs≥0.5 m/s or/and RI≥75% as criteria for differential diagnosis, the sensitivity, specificity and the accuracy of diagnosis in malignant or benign STME are greatly improved. The accuracy rate of diagnosis for STME increased from 61.5% by 2D alone to 92.3% and 80.8% when combined with CDFI (P<0.05).
