摘要
探讨口服葡萄糖耐量试验(OGTT)中空腹血糖(FPG)和2小时血糖(2hPG)的关系,以寻找最佳的FPG值作为筛选糖尿病(DM)的指标。方法对815例无DM病史者进行OGTT测试,分析FPG和2hPG之间的关系。结果和结论OGTT中,FPG和2hPG水平呈显著相关,一致性检验表明,卡帕值为0.42,有中度一致性。以WHODM诊断标准中,2hPG≥11.1mmol/L(200mg/dl)作为黄金标准,FPG≥7.8mmol/L具有100%的特异度,故可用于明确诊断;当FPG介于6.67mmol/L(120mg/dl)和7.0mmol/L(126mg/dl)之间时,它们的假阳性和假阴性之和均较小,且它们之间的差异也较小,其中以7.0mmol/L的特异性最高,故可作为诊断指标;依据ROC,FPG6.39mmol/L(115mg/dl)为最佳临界值,可作为DM筛选的标准。
bjective To analyze the relationship between fasting and 2 hours' plasma glucose in OGTT and find out the optimal value of fasting plasma glucose for the screening of diabetes mellitus.Methods The OGTT was performed in 815 subjects without history of diabetes mellitus.Statistic analysis was done.Results and conclusion There was remarkable correlation between FPG and 2hPG,and moderate consistency with Kappa value of 042.With the 2hPG≥111mmol/L as a golden standard,the specificity of FPG≥78mmol/L was 100%,which was significant for confirming a diagnosis.When the FPG was between 667mmol/L and 70mmol/L,the sum of false positive and false negative rate was less and FPG≥70mmol/L with higher specificity was significant as a diagnostic criterion. According to the ROC,FPG level of 639mmol/L was an optimal critical value.
