摘要
目的:探讨剖宫产率增高的相关因素。方法:回顾性分析自2003年1月至2008年12月间我院行剖宮手术的患者969例,记录其剖宫产率、剖宫产手术指征及构成比以及指征的变化,对剖宫产率增高的原因进行总结分析。结果:6年间剖宮产总率为31.77%;969例剖宫产中胎儿因素441例(45.51%),母亲因素183例(18.89%),头骨盆因素217例(22.39%),社会因素128例(13.21%)。2003年至2008年剖宫产中胎儿因素分别为30.18%、36.69%、44.38%、47.93%、47.34%、54.44%,呈逐渐上升趋势;近3年母亲因素有下降趋势,分别为19.53%、18.34%、18.93%;而社会因素近6年均呈逐渐上升趋势,分别为8.88%、9.48%、11.24%、13.02%、15.38%、17.75%。结论:剖宫产率不断升高的原因众多,剖宫产指征也发生很大变化,医务人员应重视产前宣教,合理掌握剖宫产指征,并引导孕妇及家属对剖宫产利弊的正确认识。
Objective： To investigate the correlation factors of increasing rate of cesarean delivery. Method： 969 cases who received cesarean delivery in our hospital from January in 2003 to December in 2008 were selected, and the rate of cesarean delivery, indication of operation constitut ratio and its changes were recorded to analyse on correlation factors of increasing rate of cesarean delivery. Result： The rate of cesarean delivery was 31.77% during 6 years. There were 441 （45.51%） cases with factors of fetus, 183 （18.89% ） cases with factors of mothers,217（22.39% ） eases with factors of head and pelvis and 128（13. 21% ） cases with social factor in the 969 cases. The rate of fetus factors increased from 2003 to 2008, and it was 30.18% ,36. 69670,44.38% ,47.93% ,47.34% and 54.44%. The rate of mother factors decreased from 2005 to 2008, and it was 19.53%, 18.34% and 18.93%. The rate of social factors increased from 2003 to 2008, and it was 8.88% ,9.48%, 11. 24670,13.02%, 15.38% and 17.75%. Conclusion： The cause of increasing rate of cesarean delivery was numerous, and the indieatio of operation changes obviously, so the medical member must grasp the indication of operation and guide the patients to know the cesarean delivery correctly.
