慢性呼吸衰竭呼吸机依赖患者的脱机方法研究

摘要 目的比较慢性呼吸衰竭呼吸机依赖患者采用不同脱机方法的脱机成功率和脱机时间。方法39例慢性呼吸衰竭呼吸机依赖患者随机分为两组,试验组19例,采用间歇性停机法;对照组20例,采用同步间歇指令通气+压力支持通气(SIMV+PSV)。比较两组患者3周内脱机成功率和脱机时间。结果试验组和对照组2周内脱机成功率分别为78.9%和35.0%,差异有显著统计学意义(P<0.01);3周内脱机成功率分别为78.9%和65.0%,差异无统计学意义(P>0.05);试验组和对照组脱机所需时间分别为(7.9±4.1)天和(15.3±6.2)天,差异有显著统计学意义(P<0.01)。结论慢性呼吸衰竭呼吸机依赖患者脱机时,采用间歇性脱机法脱机成功率较高和脱机所需时间较短,具有重要的临床价值。 Objective To compare the successful rates and weaning time of mechanical ventilation-dependency patients with chronic respiratory failure (CRF) adopting different weaning methods. Methods Thirty nine mechanical ventilation-dependency patients with CRF were randomized into the experimental group(n =19) and the control group(n =20). Patients in the experimental group using intermittent suspending respirator, while patients in the control group using pressure support ventilation plus synchronized intermittent mandatory ventilation(PSV+SIMV). After three weeks, the successful rates and the time of weaning were observed in both groups. Results After two weeks, the successful rates of weaning in the experimental group and the control group were 78.9% and 35.0% respectively, which had significantly difference between two groups (P〈0.01).Three weeks later, the successful rates of weaning in two groups were 78.9% and 65.0% respectively, which had no difference between two groups (P〉0.05). The weaning time of two groups were(7.9±4.1) days and (15.3±6.2) days respectively, which had significant difference between two groups(P〈0.01). Conclusion Compared with the method of PSV+SIMV, the method of intermittent suspending respirator has higher successful rates of weaning and less weaning time in mechanical ventilation-dependency patients with CRF.
