口腔正畸微种植体的实验研究

摘要 目的:研究微种植体在即刻正畸力影响下的支抗稳定性。方法:20枚钛合金微种植体,放置于猪上下颌颊侧根间区,立即承担200g持续的水平力。6周后测量相互受力的微种植体的稳定性,行种植体周边袋及组织学检查。结果:19枚种植体稳定性良好,能承担即刻的矫治力;有1枚微种植体松动,周围X线阴影显示种植失败征象;上下颌骨种植体均有轻度的移位,上颌种植体移位多于下颌种植体,表明种植体在受力后均有移位现象,属正常现象;组织学分析显示成功的种植体与骨组织中间没有纤维结缔组织干扰,松动的种植体周围有炎性细胞、纤维结缔组织包绕。结论:微种植体是一种可靠的正畸支抗源,与骨组织有良好生物相容性,并可即刻施力。 Objective: To study the anchorage ability and histological characteristics of micro--implants under imme- diate and continuous orthodontic loadings. Methods: Thirty titanium alloy micro--implants were placed in the interradicu- lar area of unilateral buccal side of the maxilla and mandible in a dog. A 200g force was given by nickel--titanium coil springs. After six weeks, the stability and crevicular pockets were measured, and histological evaluation was made. Results: Most micro--implants were stable and can sustain immediate orthodontic loading. One loosened sample with peri-- implan tradiolucencies was seen. The disposition was seen in the maxillary and mandibular implants. Extrusion and tip- ping were seen at the anterior part of the upper and lower jaws. Histological analysis revealed no fibrous connective tissue in the successful samples. A loose sample was placed into a tooth root with inflammation cells and fibrous tissue, and no hone tissue was seen around it. Conclusion: Micro--implants are important orthodontic anchorage, with a good biocom- patibility to bone, and can sustain immediate force.
