摘要
应用CCM能量测定系统测定老年人的REE.实测定57例,平均年龄超过69岁.实测REE男性为6.00±0.16MJ,女性4.41±0.33MJ.实测的REE与HB、Owen公式计算的BEE值比较,男性的实测REE高于HBBEE,但差异不显著(P>0.05),低于OWBEE10%(P<0.01);女性实测REE低于HBBEE和OWBEE,相差值分别是22.3%(P<0.05)和14.1%(P<0.05).老年超理想体重者不宜用单一的体重参数来作为营养状态的评定,CHI标准值%与REE实测值呈现正相关,按CHI标准值%判定本组病人57.14%存在中~重度营养不良,营养正常和有中等度、重度营养不良的实测REE分别是6.42±0.37MJ,5.88±0.31MJ,5.29±0.36MJ.
57 patients to be operated upon in our department, mean age over 69 years old ,52 males and 5 females, determined resting energy expenditure (REE) by medical graphics critical care monitor desktop analysis system (CCM system) reported herewith. The results showed that REE was 6. 00 ±0. 16MJ in males,and 4. 41±0. 33MJ kcal in females. Interestingly, the figures of REE compared with the basic energy expenditure calculated by the formula of Harris-Benedict (HRBEE) and Owen (OWBEE) the REE in males were higher than that of HBBEE, but no significant difference ( P >0. 05),and lower than that of OWBEE in 10%(P <0. 01);REE in females were both lower than those of HRBEE in 22. 30% ( P <0. 05)and OWBEE in 14. 18% (P <0. 05), respectively. Meantime, it was demonstrated that it would be better to evaluate nourishment state by CHI in elderly rather than body weight only. The results stated that the level of standard. value of CHI % were associated with the figures of REE positively, and found that 57. 14% of the cases in the series were in mild to severe malnutrition state. According to the nourishment state, the results of REE were 6. 42 ±0. 37MJ in normal nourishment group,5. 88±0. 31MJ in mild malnutrition,and 5. 29 ±0. 36MJ in severe malnutrition ,respectively.
