摘要
目的探讨联合检测甘氨酰脯氨酸二肽氨基肽酶(GPDA)、腺苷脱氨酶(ADA)、癌胚抗原(CEA)、细胞角质蛋白19片段(CYFRA21-1)对老年胸腔积液性质鉴别诊断的应用价值。方法应用电化学发光法、酶化学比色法检测经病理或临床确诊的30例结核性胸腔积液、49例瘤性胸腔积液中GPDA、ADA、CEA、CYFRA21-1含量。结果GPDA活性及阳性率在结核胸腔积液组和癌性胸腔积液组无差异(P>0.05);ADA在结核胸腔积液组的活性、阳性率显著高于癌性胸腔积液组(P<0.01);CEA、CYFRA21-1在癌性胸腔积液组的含量及阳性率明显高于结核胸腔积液组(P<0.01);GPDA、ADA、CEA、CYFRA21-1联合检测对结核性胸腔积液、癌性胸腔积液的敏感性可分别提高达到90.0%、91.8%,约登指数分别为0.847、0.864。结论GPDA、ADA、CEA、CYFRA21-1联合检测对老年人胸腔积液鉴别诊断有较高的指导意义。
Objective To explore the value of combined detection of GPDA,ADA,CEA and CYFRA21-1 in elder patients’pleural effusion. Methods The activity of GPDA,ADA,CEA and CYFRA21-1 wre investigated in 30 tuberculous benign patients and 49 malignant pleural effusion patients.Results The activity and positive rate of GPDA in tuberculous benign and malignant pleural effusion have no significance(P>0.05);ADA's activity and positive rate in tuberculous benign pleural effusion are higher than in malignant pleural effusion(P<0.01);The activity and positive rate of CEA and CYFRA21-1 in malignant pleural effsion are higher than in tuberculous benign pleural effusion(P<0.01).The sensitive rate of combining detection of GPDA,ADA,CEA and CYFRA21-1 in tuberculous benign,malignant pleural effusion are 90.0%,91.8% respectively which are highly developed; the Youden’s index are 0.847,0.864 respectively. Conclusion Combining detection of GPDA,ADA,CEA and CYFRA21-1 is useful in differential diagnosis of elder patients’tuberculous benign and malignant pleural effusion.
