摘要
【目的】评价新柏士液基薄层细胞学检查（TCT），人乳头状瘤病毒（HPV）高危型检测（HR—HPV）对宫颈病的筛查价值。【方法】对门诊858例TCT异常（TBS系统诊断≥ASC—US）的患者，同时行HPV高危型检测，配合阴道镜检查并取活检，以病理学为标准对结果进行分析。[结果]858例液基细胞细胞学异常中与病理组织学检查的符合率为：LSIL76．2％（112／147），HSIL90．1％（136／151），宫颈癌100％（9／9）。细胞学异常858中，HPV阳性率分别为ASC34．7％（191／551），LSIL78．2％（115／147），HSIL91．4％（149／163），宫颈浸润癌88．9％（8／9）。858例患者阴道镜组织活检病理学诊断结果的HR—HPV阳性率分别为：慢性宫颈炎17．2％（67／390）、CIN ⅠHPV77．3％（225／291）、CINⅡCINⅢ92．7％（140／151）宫颈癌92．9％（13／14）。【结论】TCT检查，HPV检测是宫颈病变筛查的有效方法，二者结合提高宫颈病变及宫颈癌的检出率。
[Objective] To evaluate the significance of thinprep liquid-based cytology test （TCT） and the high risk-human papillomavirus （HR-HPV） test in the screening of cervical lesion. [Methods]A total of 858 cases with abnormal TCT were examined for HR-HPV by hybrid capture Ⅱ ,then received biopsy with colposcopy. [Results]Among the 858 cases, the coincidence between the findings of TCT and pathology was 76.2% （112/147） for LSIL, 90.1%（136/151） for HSIL and 100%（9/9） for cervical cancer. In the total cases, the positive rate of HR-HPV of ASC, HSIL and cervical caner was 34. 7% （191/551） , 78. 2% （115/147）, 92.7 % （ 140/151） and 88.9 % （8/9）, respectively. Among pathologic examination by electronic colposcopy revealed that the positive rate of HR-HPV in chronic cervicitis, CIN Ⅰ , CIN Ⅱor CINⅢ and invasive cancer of cervix was 17.2%（67/390）, 77.3%（225/291）, 92.7%（140/151） and 92.9%（13/14）, respectively. [Conclusion]TCT and HR-HPV detection are effective methods for screening cervical lesion. Cytology combined HR-HPV test can increase the diagnosis of cervical lesion.
