摘要
目的：探讨卵巢生殖细胞肿瘤的ＣＴ表现特点，提高ＣＴ诊断的准确性。材料与方法：回顾性分析１９９０年以来经我院手术病理证实的２０例２２个卵巢生殖细胞肿瘤的ＣＴ及Ｂ超影像资料，结合病理讨论其ＣＴ表现特点。结果：本组卵巢生殖细胞肿瘤中，畸胎瘤１７个，内胚窦瘤２个，无性细胞瘤３个。１７个畸胎瘤中，１６个见脂肪密度区。１４个成熟性囊性畸胎瘤中，１２个边缘光滑，９个有浮球征，１个见脂液平面，５个囊壁有钙化点，１个块内见牙齿状影。３个恶性畸胎瘤均为实质性肿块，肿块边缘不规则，内见散在钙化斑点，并且较多盘曲的带状略低密度影，镜下为脑组织。１７个卵巢畸胎瘤均作了Ｂ超检查，其敏感性为９４．１％，特异性为６４．７％，而ＣＴ均为１００％。内胚窦瘤肿块以实质性为主，间杂有小片状低密度区。无性细胞瘤边缘光滑，１个呈实质性肿块，密度均匀，另２个呈囊实性肿块，囊实性成分交界处边缘模糊。结论：卵巢畸胎瘤和内胚窦瘤的ＣＴ表现具有各自的特点，ＣＴ能作出正确诊断，尤其是对畸胎瘤，与Ｂ超相比，ＣＴ是更好的检查方法。
Objective: To study the CT features of ovarian germ cell tumors and to improve the CT diagnostic accuracy.Materials and Methods:22 surgical and pathologic proved ovarian germ cell tumors in 20 cases with their CT and US images were analysed retrospectively. The characteristics of their images were discussed correlated with their pathology.Results:22 germ cell tumors composed of 17 teratoma, 2 endodermal sinus tumor and 3 dysgerminoma. Fat attenuation were found in 16 of 17 teratoma. In benign cystic teratoma ([WT5”BX〗n=14), we found 12 were smoothly bordered, 9 had a round mass in the cyst, 1 had fatfluid level, 6 had calcification or tooth. 3 malignant teratoma presented as solid mass with calcification and irregular border. Some curved bandlike low attenuation area were also found on CT, which were brain tissue confirmed by pathology. 17 teratoma were all demonstrated by CT. In which only 11 were demonstrated by US. 2 endodermal sinus tumor presented as mainly solid masses with some small pieces of hypodense area. 3 dysgerminoma were smoothly bordered, 2 of them were cysticsolid and the other one was homogeneously solid.Conclusion: Ovarian teratoma and endodermal sinus tumor had their characteristic CT features. They could be diagnosed accurately by CT. Compared with US, CT is a better image modality, particularly for teratoma.
