实时三维超声心动图预测心脏移植后排异反应的应用价值 被引量:6

Application value of real-time three-dimensional echocardiography for forecast rejection after heart transplantation

摘要 目的探讨实时三维超声心动图17节段时间-容积曲线参数指标预测心脏移植后排异反应的价值。方法应用PhilipsiE33超声显像仪,X3—1探头对60例心脏移植无排异患者、10心脏移植排异患者、40例正常对照者进行实时三维超声心动图检查。结果心脏移植排异组的17节段时间-容积曲线参数指标Tmsv16-SD、Tmsv12-SD、Tmsv6-SD、Tmsv16-Dif、Tmsv12-Dif、Tmsv6-Dif以及Tmsv16-SD(%)、Tmsv12-SD(%)、Tmsv6-SD(%)、Tmsv16-Dif(%)、Tmsv12-Dif(%)、Tmsv6-Dif(%)较正常组及心脏移植无排异组高,差异有统计学意义(均P〈0.05),17节段时间-容积曲线参数指标Tmsv16-SD(%)、Tmsv12-SD(%)、Tmsv6-SD(%)预测心脏移植后排异的敏感性及特异性较高。结论实时三维超声心动图17节段时间-容积曲线参数指标能快速预测心脏移植术后早期排异反应。 Objective To investigate the value of real-time three-dimensional echocardiography 17 regional volume curves index in patients with heart transplantation. Methods Sixty patients with heart transplantation (no rejection), ten patients with heart transplantation (rejection), forty subjects with normal left ventricular(LV) function were examined by Philips iE33 with a X3-1 probe. Results Parameter index of 17 segment time to minimal systolic volume in patients with heat transplantation (rejection) were more than those in patients with heart transplantation (no rejection) and in subjects with normal LV function(all P 〈0.05). Parameter index of 17 segment time to minimal systolic volume were not significantly different in patients with heart transplantation (no rejection) compared with subjects with normal LV function. Parameter index of Tmsv16-SD (%), Tmsvl2-SD (%) and Tmsv6-SD (%) show higher sensitivity and specificity for forecast rejection after heart transplantation. Conclusions Real-time three-dimensional 17 regional volume curves index can fast forecast early rejection after heart transplantation.
