成分分离技术治疗严重污染条件下腹壁切口疝和缺损的初步研究(附七例报告) 被引量:10

摘要 目的总结并分析成分分离技术在严重污染条件下对腹壁切口疝和缺损的治疗效果。方法回顾分析我院自2004年6月至2007年6月3年间以成分分离技术对7例严重污染条件下的腹壁切口疝和缺损的治疗经验。结果7例患者均顺利完成手术,术后1例腹壁血肿,1例局部小面积皮肤坏死,经保守治疗治愈;全部患者随访12—36个月,1例因结肠癌复发转移而死亡,其余6例患者无感染、疼痛等局部不适,无复发。结论当创面污染严重,以合成材料行无张力修补存在禁忌的情况下,应用成分分离技术进行腹壁重建不失是一种被选方案。 Objective To summarize and investigate the therapeutic effectiveness to the abdominal wall incisional hernia and defect with "components separation technique" in the setting of severe contamination. Methods The clinical data of 7 patients with the abdominal wall incisional hernia or defect in the setting of severe contamination were reviewed. Results The complications happened in 2 patients: hematoma in 1 ,skin necrosis in 1. After a fellow- up period of 12 to 36 months, 1 died of recurrence and metastasis of colon cancer and the remaining 6 patients had no local discomfort, and no recurrence was found. Conclusion The "components separation technique" is an alternative for the reconstruction of abdominal wall under severely contaminated conditions in which the use of prosthetic material is maybe contraindicated.
