日本血吸虫病血清学诊断试验的Meta分析 被引量:3

摘要 目的通过诊断试验Meta分析,综合评价间接血凝集试验(IHA)和酶联免疫吸附试验(ELISA)对日本血吸虫病的诊断效果。方法通过检索Pubmed、SCI、CNKI、Proquest、维普等数据库,获得公开发表的关于日本血吸虫血清学诊断试验的文献,提取相关数据。采用SAS8.02进行统计分析,利用Excel2003绘制SROC曲线图。结果15篇文献符合纳入标准,Meta分析稳健法结果显示IHA法的综合灵敏度为0.721,诊断优势比为6.686。ELISA法的综合灵敏度为0.633,诊断优势比为2.963。结论两种血清学诊断试验(IHA和ELISA)对日本血吸虫病的诊断准确度中等,IHA略高于ELISA,但差异无统计学意义。两种诊断方法对日本血吸虫病均具备一定的诊断价值。 Objective To investigate the concerned tests efficacy of enzyme-linked immunosorbent assay (ELISA) and indirect hemagglufination assay (IHA) in the diagnosis of schistosomiasis japonica. Methods Original articles were searched in the Pubmed, Science Citation Index Expanded ( SCI ), CNKI, Proquest and VIP Information. Analysis was undertaken using SAS Software, version 8.02 and SROC ( summary receiver operating characteristic) curve was given by Excel 2003. Results Fifteen articles were included in our research. The results of meta-analysis revealed that the accuracy of IHA ( TPR = 0. 721 ) with DOR ( 6. 686 ) is higher than that of enzyme-linked immunosorbent assay ( TPR = 0. 633, DOR = 2. 963 ). Conclusion Both IHA and ELISA can be used in diagnosis of schistosomiasis japonicum for the accuracy of them are moderate. The result of Z test can not prove that a significant difference exist between the two methods in screening of schistosomiasis.
