彩色多普勒超声与超声造影对肝脏良性局灶性病变的鉴别诊断 被引量:2

Differential Diagnosis of Benign Focal Liver Lesions by Contrast Enhanced Ultrasound and Color Doppler Ultrasonography

摘要 目的:探讨彩色多普勒超声与超声造影对肝脏良性局灶性病变的鉴别诊断价值。方法:对经病理、增强CT/MRI证实的38例患者42个肝脏良性局灶性病变进行彩色多普勒血流显像检查,然后进行实时超声造影检查。结果:①本组中良性病灶42个,其中血管瘤20个,局灶性增生结节9个,肝硬化增生结节6个,局灶性脂肪缺失6个,炎性结节1个。②彩色多普勒超声诊断良性局灶性病变29个(29/42,69.05%),超声造影后诊断良性局灶性病变38个(38/42,90.48%),二者之间诊断良性病变符合率具有显著性差异(P<0.05)。③超声造影中,肝脏血管瘤,局灶性增生结节分别具有向心性增强(18/20,90%),轮辐状增强(9/9,100%)这一特征性增强模式,可作为诊断的依据。结论:超声造影对肝脏良性局灶性病变的诊断优于彩色多普勒超声,二者互相结合有助于对肝内良性局灶性病变的鉴别诊断。 Objective To explore the clinic value of differential diagnosis of benign focal liver lesions by contrast enhanced ultrasound and color Doppler ultrasonography. Methods Thirty-eight patients with 42 benign focal liver lesions were examined by CnTIUS and color doppler ultrasonography, and all were confirmed by pathology, enhanced CT or MRI. Results In 42 benign focal liver lesions there were 20 hemangiomas, 9 focal nodular hyperplasias, 6 hyperplastic nodules, 6 focal fatty sparings and 1 inflammatory nodule. The diagnostic rate of benign focal liver lesions had remarkable significance between color doppler ultrasonography 69.05% (29/42) and contrast enhanced ultrasound 90.48% (38/42) (P 〈 0.05). The typical enhancement pattern can be diagnostic basis which showed centripetal fill-in enhancement from periphery enhancement in hemangioma and central spoke contrast enhanced ultrasound. Conclusion wheel-shaped enhancement in Contrast enhanced ultrasound focal nodular hyperplasias by is better than color Doppler ultrasonography in diagnosing benign focal liver lesions, the two technic combined together could be helpful in differential diagnosising benign focal liver lesions.
