摘要
目的探讨不同剂量乙肝疫苗接种对早产儿强或无(弱)应答的影响以及免疫球蛋白(IgG、IgM、IgA)、补体(C3、C4)的变化。方法90例早产儿根据接种剂量随机分为:A组三剂均为5μg,B组三剂均为10μg,免疫程序均为0、1、6个月三针法,每组45例。比较两组乙肝疫苗抗体阳转率、强应答率差异,分析比较强应答与弱应答时免疫球蛋白水平的变化。结果各组患儿未见明显局部与全身反应,均顺利完成临床试验。B组免疫接种后IgG、IgM、IgA、C3、C4水平与A组相近(P>0.05)。A、B两组强应答早产儿的IgM、IgA、C3水平明显高于无(弱)应答早产儿IgM、IgA、C3水平(P<0.01)。B组强应答早产儿IgG、IgM、IgA、C3、C4水平明与A组强应答早产儿的IgG、IgM、IgA、C3、C4水平比较,差异无统计学意义(P>0.05)。B组乙肝疫苗抗体阳转率明显高于A组(P<0.05)。结论高剂量乙肝疫苗可提高早产儿乙肝疫苗接种的乙肝疫苗抗体阳转率,强应答的早产儿有较无(弱)应答早产儿有更好的体液免疫功能。
Objective To investigate the immune response of prematures after vaccination of HBV vaccine and changes in humoral immunity. Methods 90 prematures were devided into 2 groups ：prematures in group A （n=45 ） received recombinant beer yeast hepatitis B vaccine （with doses of 5 μg, 5 μg and 5 μg） and prematures in group B （n=45） received the same vaccine （with doses of 10 μg, 10 μg and 10 μg ） in following 0,1 and 6 months. After immunization, local and systemic reaction were observed and recorded during 72 h. After 6 months, anti-HBs titre and seroconversion rate, IgG, IgM, IgA, C3 and C4 were measured. Results There was no obviously local and systemic reaction seen in all groups in which all tests were finished successfully. Levels of IgM, IgA and C3 of Groups A and B with strong immune response were higher than those with none （low） immune response （P 〈 0.05 ）. Seroconversion rate of Group B was higher than that of Group A （P 〈 0.05 ）. Conclusions The higher dosage of vaccination of HBV vaccine may enhance the effect of premature＇s immune response, and prematures with strong immune response have a better humoral immune function.
