摘要
目的探讨带旋髂深血管蒂髂骨瓣移植治疗儿童股骨头缺血性坏死的疗效。方法回顾性分析1996年7月至2005年12月应用带旋髂深血管蒂髂骨瓣移植治疗29例儿童股骨头缺血性坏死患者的临床资料，Catterall分型：Ⅰ型4例，Ⅱ型10例，Ⅲ型13例，Ⅳ型2例。手术采用带旋髂深血管蒂髂骨瓣移植、滑膜切除术，术后单髋“人”字石膏固定髋关节于外展、内旋位3个月，3个月后逐步功能锻炼。结果随访1～4年，平均2年。根据评定标准，本组29例，优17例，良8例，可2例，差2例，优良率86．2％。结论带旋髂深血管蒂髂骨瓣移植治疗儿童股骨头缺血性坏死具有取材方便、操作较简便、效果可靠的特点，是目前治疗儿童股骨头缺血性坏死的一种有价值的手术。
Objective To document therapeutic effect of iliac bone flap with deep circumflex iliac vessel pedicle grafting for avascular necrosis of femoral head in children. Methods Twenty - nine children with avascular necrosis of femoral head, that treated with iliac bone flap transplantation with deep circumflex iliac vessel pedicle from July, 1995 to December, 2005 ,were analyzed respectively. The staging methods of radiographs adopted the Catterall＇ s classification, among them Catterall Ⅰ4 cases, Catterall Ⅱ 10 cases, Catterall Ⅲ 13 cases, Catterall Ⅳ 2 cases. All patients were treated with iliac bone flap transplantation with deep circumflex iliac vessel pedicle and synovectomy, coxa spica plaster immobilized 3 months after operation, 3 months later functional exercise were done gradually. Results Twenty - nine cases were followed up from one year to four years （average two years）, and the excellent rate was 86.2% in total cases. Conclusions Treatment of avascular necrosis of femoral head in children by iliac bone flap transplantation with deep circumflex iliac vessel pedicle ,which is convenient to take the material of reconstruction and easy to handle ,is a valuable operation type at present.
