摘要
13只犬肺切除后支气管残端采用液氮行冷冻处理,观察7天~7个月(平均3.5个月)。结果见冷冻的支气管残端均有坏死,随观察时间的延长逐渐形成纤维瘢痕。全组均未发生残端破裂或支气管胸膜瘘。其原因是冷冻坏死发生缓慢,纤维组织对冷冻的耐受性较强,使坏死组织不致很快脱落;肺切除手术污染轻,冷冻的支气管残端无感染溃烂;覆盖支气管残端的胸膜因冷冻而较快发生粘连和纤维组织增生。
Aim:This experimental study was conducted to examine the effect of cryothermia on healing of bronchial stump.Material and method:Thirteen healthy adult dogs were studied.Pneumonectomy or lobectomy was performed in all experimental animals.The bronchial stump was closed with ligation.Liquid nitrogen was applied to the bronchial stumps twice.Cryothermia was terminated immediately when the temperature reached 40℃.After each cryothermia exposure,The stumps were allowed to rewarm to room temperature.Specimens of bronchial stump were taken at end of observation from 7 days to 7 months(mean,3.5months).Results:One animal died from empyema 7 days postoperatively.Pathological study suggested that necrosis of frozen bronchial stump occurred in all specimens and fibrosis or scar formation increased with time.No rupture of bronchial stump or bronchopleural fistula was found.Conclusion:The results of this study suggest the use of cryothermiatherapy in bronchial operation appears to be safe and effective in spite of adverse local reactions.However,further investigations are needed to evaluate its real value in promoting healing of brouchial stump.
