摘要
目的探讨乙肝病毒表面抗原(HBsAg)浓度与α-L-岩藻糖苷酶(AFU)及甲胎蛋白(AFP)的相关性。方法收集240例HBsAg阳性病例的血清,根据HBsAg定量的不同浓度分为1组(0.5ng/ml~150ng/ml)、2组(151ng/ml~300ng/ml)、3组(301ng/ml~450ng/ml)、4组(>450ng/ml),每组60例,正常对照组60例,分别检测AFU、AFP,对其结果进行分析。AFU检测采用速率法,HBsAg检测采用时间分辨免疫荧光法,AFP检测采用化学发光法。结果1组的AFU与AFP呈正相关(r=0.310,P<0.05),2组的AFU与AFP也呈正相关(r=0.298,P<0.05),3组和4组的AFU与AFP之间无相关性(分别r=0.298,r=0.067,P>0.05);各HBsAg浓度组与对照组AFU比较差异无统计学意义(F=1.719,P>0.05),而与对照组AFP比较差异有统计学意义(F=3.000,P<0.05);各组AFU阳性率随HBsAg浓度升高无升高趋势(χ2=1.44,P>0.05),而AFP阳性率随HBsAg浓度升高呈升高趋势(χ2=8.28,P<0.005)。结论对HBsAg高浓度的HBV感染者同时检测AFP和AFU对观察肝脏损害的演变过程有重要的临床价值。
Objective To explore the correlations of the concentration of HBsAg with the levels of AFU and AFP. Methods Sera of 240 patients positive for HBsAg were collected.and the serum samples were assigned into four groups (n= 60 in each group) based the different concentrations of HBsAg: group 1 (0.5ng/ml-150ng/ml),group2 (151ng/ml-300ng/ml), group3(301ng/ml-450ng/ml),group4(〉450ng/ml)and 60 normal serum samples as control group. The leveld of AFU and AFP were determined and analysed. Velocity method was used to detect AFU. Timedifference immumofluorescence method was used to test HBsAg and chemil method for AFP. Results Positive correlation existed between AFU and AFP in group 1 and group 2 (groupl:r=0.310,P〈0.05,gruup2:r=0.298,P〈0.05).There was no correlation between AFU and AFP in group 3 and group 4 (respectivelyr=0.298,r=0.067,P〉0.05). The levels of AFU in groups with different eoncentration of HBsAg showed no statistically significant difference compared with control group (F=1.719,P〉0.05), while the level of AFP was statistically significant difference (F=3.000,P〈0.05) . The positive rate of AFU did not increase with the increase of concentration of HBsAg (X^2 =1 .44 , P 〉0.05 )in these groups but that of AFP did increase along with the increase of concenration of HBsAg (X^2=-8.28,P〈0.005). Conclusion The determination of levels of AFU and AFP in those with high concentrations of HBsAg at the same time is of important cilinical value for observation the developmental process of liver impairment.
