摘要
目的探讨口服中药联合稀碘伏灌肠对促进胃肠功能的恢复、减轻肝肾功能损害、减少并发症以及降低住院费用、住院天数的作用。方法将2007年7月～2008年7月我院腹部手术患者100例随机分为实验组和观察组，每组50例。实验组患者入院后，即服用中药煎剂茵陈小承气汤，并行生理盐水碘伏液（含有效碘0．025％）500ml每天保留灌肠一次，手术前夜用生理盐水碘伏液清洁灌肠；术后48h后经胃管缓慢注入茵陈小承气汤，闭管2h，每天2次，行生理盐水碘伏液500ml每天保留灌肠一次，连续5d。对照组按腹部手术常规处理。结果两组在促进胃肠功能的恢复、减轻肝肾功能损害、促进早期肠蠕动、减少并发症、降低病人住院费用和天数等方面差异有显著意义。结论茵陈小承气汤在术后恢复胃肠功能方面效果确切，生理盐水碘伏液灌肠可以预防肠道细菌移位所致的腹腔感染。
Objective To study the effect of oral Chinese medicine combined by dilute lodophor enema for enhance the recovery of gastrointestinal function, and reducing complication and hospitalization cost. Method 100 patients with abdominal surgery form July, 2007 to July ,2008 were randomly divided into experimental group and control group. Patients in experimental group was administrated the Chinese medicine Yinchen XiaoChengQi decoction corn bined by dilute Saline liquid iodine （effective iodine containing： 0. 025 % ） 500 ml retention enema once a day, the di lute saline solution iodophor cleaning enema was conducted before surgery. The Yinchen Xiao Cheng Qi decoction was slowly injected by gastric tube in experimental group 48 hours after surgery, and closed tube 2 hours twice a day. Dilute lodophor combined by normal saline solution 500 ml retention enema once a day for 5 days. Patients in control group was dealing with abdominal conventional treatment. Result There was significant difference in the pro motion of gastrointestinal function, reduce liver and kidney damage, promoting early peristalsis, reducing complica- tions and reducing hospitalization costs between experimental group and control group. Conclusion Yinchen Xiao ChengQi decoction is effective for the treatment of postoperative recovery of gastrointestinal function, the better prognosis. Dilute iodine enema can prevent the infection of abdominal cavity caused by abdominal bacteria shift.
