摘要
目的:软骨肉瘤是常见的骨恶性肿瘤之一。其形态多变、生物学行为复杂,对软骨瘤和高分化软骨肉瘤,只凭病理组织学特征鉴别它们通常是很困难的,并且软骨肉瘤组织学分级与其生物学行为时常不一致。因此,需要更为客观可靠的指标来协助诊断,指导软骨肉瘤的分级和估计软骨肉瘤的预后。肿瘤细胞周期的研究是当前肿瘤研究热点之一,肿瘤细胞周期受一系列细胞周期调节因子控制,包括G1期细胞周期素(cyclins)和相应的细胞周期素依赖激酶(cyclin-dependentkinases,CDKs),CDKs活性又受一系列细胞周期素依赖激酶抑制物(cyclin-dependentkinaseinhibitors,CDKIs)控制。p27是新近发现的CDKIs家族之一,在细胞周期增殖和分化调控中起非常重要作用。p27蛋白主要抑制G1期cyclinE-CDK2复合物及cy-clinD-CDK4/6复合物活性,它过度表达可使细胞周期停滞在G1中期。软骨肉瘤p27表达国内外尚未见报道。检测软骨肉瘤中p27表达,从而探讨p27表达与软骨肉瘤组织分级和DNA倍体的关系及其对软骨肉瘤的诊断和预后判断是否具有应用价值十分必要。方法:36例经典软骨肉瘤未脱钙?
Purpose To investigate the practical significance of p27 expression in the diagnosis and prognostic assessment of chondrosarcomas. Methods p27 expressions were sutdied in paraffinembdded sections of the conventional chondrosarcoma by means of an immunohistochemical(SP) assay, and DNA ploidy were measured by flow cytometry. Results Chondrosarcomas with high histological grade showed a bias toward an increase in p27 expression, while normal cartilaginous tissues and benign cartilaginous tumors gave negative results. There was a statistical significance (t=206, P<005) between p27 expressions of chondromas and gradeⅠchondrosarcomas.In normal cartilaginous tissues, benign cartilaginous tumors and low grade (grade Ⅰ and grade Ⅱ) chondrosarcomas were mainly diploid, and in high grade ( gradeⅢ) chondrosarcomas were aneuploid. The higher p27 protein expressed, the more DNA aneuploid presented. p27 expression showed a significant direct correlation with DNA aneuploid (r=0925, P<005). Conclusions p27 protein will probably become a marker in differentiating chondromas from grade Ⅰ chondrosarcomas and a useful prognostic index of chondrosarcoma, and it may also be of some guiding significance in grading chondrosarcoma.
