多层螺旋CT对女性盆腔囊性病变的鉴别诊断 被引量:11

摘要 目的提高多层螺旋CT诊断女性盆腔囊性肿块的正确性。方法收集明确诊断的女性盆腔囊性肿块84例共102个病灶,所有病例行多层螺旋CT平扫、增强扫描及多平面重建,并对其CT表现作回顾性分析。结果102个囊性包块,良性86个,恶性16个。CT诊断定位正确80例(96.1%),定性正确69例(82.1%)。其中卵巢囊性畸胎瘤13例13个病灶;卵巢囊腺瘤11例15个病灶;卵巢囊腺癌5例9个病灶;卵巢转移瘤3例4个病灶;卵巢内膜异位囊肿12例16个病灶;卵巢囊肿19例22个病灶;输卵管积水或积脓5例7个病灶;子宫内膜癌3例;盆腔囊性感染性包块8例;其它5例。结论多层螺旋CT对女性盆腔囊性肿块定位及定性诊断具有较高价值。 Objective To improve the diagnostic accuracy of cystic masses in the female pelvis with multi-slice spiral CT. Methods CT finding of 84 patients with 102 proved cystic masses in the female pelvis were retrospectively analyzed. All patients underwent multi-slice spiral CT scan and multi-planar reconstruction(MPR). Results Of 102 cystic masses, 86 were benign,16 were malignant. The accurate number of CT orientational diagnosis was 80 cases (96.1%), the accurate number of CT diagnosis of natural decision was 69 cases (82.1%). All cystic masses included following diseases: ovarian cystic teratoma(n=13, 13 cases), ovarian cystadenoma(n=15, 11 cases), ovarian eystadenocarcinoma(n=9, 5 cases), ovarian metastasis tumor(n=4, 3cases), ovarian endometrial cyst(n=16, 12 cases), ovarian cyst (n=22, 19 cases), tubal abscess or hydrosalpinx(n=7, 5 cases), carcinoma of endometrium (n=3), pelvic cystic infected mass(n=8), and miscellaneous(n=5). Conclusion Multi-slice spiral CT scan is a valuable means for the orientational diagnosis and diagnosis of natural decision of cystic mass in female pelvis.
