β-磷酸三钙生物可吸收融合器在截骨术中的应用

摘要 目的观察β-磷酸三钙(β-TCP)生物可吸收融合器在截骨矫形术中的应用效果。方法对12例(14处)因先天发育不良、创伤或感染造成踝关节、膝关节内外翻畸形的患者行张口截骨内固定手术,截骨间隙植入自行研制的β-TCP生物可吸收融合器以促进骨融合。术后通过X线及CT对截骨处骨愈合情况及生物可吸收融合器的形态变化进行随访观察,并对患者术后患肢功能和并发症进行随访评估。结果术后随访2~12个月(平均10.4个月)。术后所有患者患肢力线恢复正常,14处畸形平均矫正角度为12.5°(10°~21°),随访期内无矫正角度丢失。11例(13处)截骨处达到骨融合,13处平均融合时间4.6个月(3—6个月);1例随访2个月,有部分融合。CT及三维重建显示术后12周时所有截骨间隙均有不同程度融合,β-TCP融合器中心多孔部分大部分吸收,为骨结构替代,而外层致密环多有不连续或断裂,部分崩解。结论采用β-TCP生物可吸收融合器可以有效促进截骨矫形术后的骨融合。 Objective To observe the role of β-tricalcium phosphate (β-TCP) bioabsorbable fusion cage in osteotomy. Methods Twelve patients (14 areas) with knee or ankle deformity secondary to dysplasia, trauma or infection received opening-wedge osteotomy, β-TCP bioabsorbable fusion cages were implanted into the wedge gap after osteotomy to improve the bone healing. X ray and CT examinations were employed to analyze the bone healing and degradation of cages. The complications and limb function were recorded. Results The time of followed-up was 2 to 12 months, with an average of 10.4 months. All the limb alignments were recovered with a mean correction angle of 12.5° (10° to 21°). No loss of correction angle was found during the followed-up. Eleven cases of 13 areas got bony fusion, with a mean time of fusion of 4.6 months (3 to 6 months). Partial fusion was found in one case after follow up for 2 months. It was revealed by CT and three-dimensional reconstruction that there was fusion to some extent in the opening wedge 12 weeks postoperation, and the porous core of cage was absorbed and replaced by new bone formation. The peripheral dense circle was broken and in some cases it was collapsed. Conclusion β-TCP bioabsorbable fusion cage can effectively enhance the bone healing after osteotomy.
