摘要
目的:探讨电子阴道镜量化评分系统(RCI评分)对宫颈病变的诊断价值。方法:对2006年4月～2008年3月我院宫颈液基薄层细胞学检查(TCT)结果为不典型鳞状上皮细胞(ASCUS)及以上分型的1 936例患者进行电子阴道镜检查。对图像异常者采用RCI评分评估宫颈病变的程度,并在阴道镜下宫颈多点活检,以RCI评分诊断与组织病理学诊断进行对比分析。结果:1 936例检查对象中阴道镜图像异常1 031例,RCI评分诊断与病理诊断完全符合率为73.52%,包括相差一个级别在内符合率为99.03%,其中诊断HPV/CINI完全符合率为70.86%,诊断CINⅠ～Ⅱ完全符合率为76.51%,诊断CINⅡ～Ⅲ完全符合率94.23%,诊断浸润癌完全符合率为96.30%。结论:电子阴道镜量化评分系统(RCI评分)诊断宫颈病变较准确可靠,可预示子宫颈病变的严重程度,有很好的临床应用价值。
Objective： To examine the diagnosis value of E - vagina quantization grading system （ RCI grading） on cervix patholog- ical change. Methods： Electro vaginoscopy was taken on 1 936 cases which showed the result of atypical squamous cell and division in the cervical liquid - base thin layer cytologic examination （TCT） from April 2005 to March 2008 in this hospital. RCT grading was used to eval- uate the degree of cervical pathological change for those with abnormal image, and biopsy of multi - points of cervix taken under vaginoscope, contrastive analysis made between RCI grading and histopathology diagnosis. Results： Among 1 935 subjects, 1 031 subjects showed abnormal image under vaginoscope, the complete conformation rate between RCI grading and pathological diagnosis was 73. 52%, if difference of 1 grade included, the conformation rate was99. 035. Complete conformation rate was70. 86% for diagnosing HPV/CIN Ⅰ , 76. 51% for diagnosing CIN Ⅰ -Ⅱ, 94. 23% for diagnosing CINⅡ -Ⅲ, 96. 30% for diagnosing infiltrative cancer. Conclusion： E -vagina quanti- ration grading system （ RCI grading） is precise and reliable in diagnosing cervical pathological change, able to predict the severity of cervical pathological change and therefore has promising clinical application value.
