摘要
目的探讨应用活体角膜共焦显微镜(CM)观察角膜缘干细胞(LSC)龛环境与角膜缘基底细胞群活力和数量在正常人群中的情况。方法横断面研究。观察对象为2007年3至12月复旦大学附属眼耳鼻喉科医院眼科选取的社区正常人志愿者120名,并用随机数字表法选择单眼的眼别,入选标准为无手术、外伤、感染及角膜接触镜配戴史,裂隙灯显微镜检查角膜透明,眼底镜检查无异常。按照年龄段进行分组,A组为0-岁,B组为20-岁,C组为40-岁,D组为60~79岁,每组30例。使用CM观察研究对象的角膜缘,记录图像并分析各组Vogt栅栏存在情况、计量角膜缘基底细胞大小,对LSC龛环境质量和细胞增殖活力进行评价。结果各组角膜缘组织结构具有不同形态特点。A组29只眼存在Vogt栅栏,占96.7%;1只眼未查见Vogt栅栏,占3.3%。B组29只眼存在Vogt栅栏,占96.7%;1只眼未查见Vogt栅栏,占3.3%。C组21只眼存在Vogt栅栏,占70.O%;5只眼未查见Vogt栅栏,占16.7%;4只眼存在萎缩的Vgot栅栏,占13.3%。D组10只眼存在Vogt栅栏,占33.3%;17只眼未查见Vogt栅栏,占56.7%;3只眼存在萎缩的Vgot栅栏,占10.0%。A组角膜缘上皮基底细胞平均大小为(9.7±1.0)um,B组为(10.7±1.5)um,C组为(10.6±1.2)um,D组为(12.2±1.4)um。结论在正常人群中,角膜缘LSC龛环境存在不同特点;同时,角膜缘基底细胞活力和数量也存在相应变化。
Objective To demonstrate the variations in the niche of limbal stem cells and the limbal basal epithelial cells in vivo by confocal microscopy. Methods One hundred and twenty eyes of 120 healthy subjects were enrolled in this study after a routine slit-lamp examination. They were divided into four groups according to the age: group A (0-),group B (20-),group C (40-) and group D (60-79). Confocal microscopic examination in vivo was performed and the images of the inferior limbus were recorded. The morphologic features of each group were analyzed and the average size of limbal basal epithelial cells of each group were measured. Results The morphologic features of limbus varied among groups. 96.7% (29/30) of group A showed clear Vogt Palisades,while 3.3% (1/30) showed no Vogt Palisades. 96. 7% (29/30) of group B also showed clear Vogt Palisades,while 3.3% (1/30) showed no Vogt Palisades. 70. 0% (21/30) of group C showed clear Vogt Palisades, while 13.3% (4/30) showed atrophic Vogt Palisades and 16. 7% (5/30) showed no Vogt Palisades at all. 33.3% (10/30) of group C showed clear Vogt Palisades, while 10. 0% (3/30) showed atrophic Vogt Palisades and 56. 7% ( 17/30 ) showed no Vogt Palisades at all. The average size of limbal basal epithelial cells in each group was (9. 89± 1.12),(10.65± 1.45),(10.70 ± 1.39) and (12.22 ± 1.42) um in sequence. Conclusions There is varitions in the microenvironment of niche in human limbus. Meanwhile, limbal basal epithelial cells showed different quantity and cell size, demonstrating varied proliferative potential.
