摘要
目的探讨子宫内膜增生过长的超声表现,为临床诊断提供依据。方法回顾性分析经病理确诊的56例子宫内膜增生过长患者的临床及超声表现资料。结果所有患者子宫肌层回声均质。26例子宫大小正常。8例年龄〈40岁者,病理诊断单纯增生过长6例,腺囊样增生过长2例。40-49岁者29例中,子宫形态饱满者9例,子宫增大者8例,病理诊断单纯增生过长25例,不典型增生1例,子宫内膜腺癌3例。50-59岁者17例中,子宫形态饱满者5例,子宫增大者6例;内膜均匀增厚者11例,增厚子宫内膜回声不均质者6例;病理诊断单纯增生过长13例,腺瘤样增生过长2例,子宫内膜腺癌2例。〉60岁者2例中,子宫形态饱满者1例,子宫增大者1例,2例均表现为内膜增厚,回声不均;病理诊断单纯增生过长1例,腺囊样增生过长1例。56例患者中,一侧卵巢探及囊样暗区者31例（占55.36%）,暗区直径〈3.5 cm,双侧卵巢探及囊性暗区者8例（占14.29%）,暗区直径〈2.9 cm。结论B超能准确反映子宫肌层及内膜的变化,是诊断子宫内膜增生过长的有效方法之一。
Objective To explore the manifestations of Swiss cheese endometrial hyperplasia so as to provide diagnostic basis.Methods The clinical and ultrasonic data of 56 pathologically confirmed cases of Swiss cheese endometrial hyperplasia were analyzed retrospectively.Results All cases showed homogeneous myometrial echo.26 cases had normal uteri.8 cases were under 40.Pathological diagnoses discovered 6 cases of simple hyperaplasia and 2 cases of sac-like gland hyperaplasia.29 cases were between 40-49.Of them,9 cases had normal uteri and 8 cases had hyperplasia.25 cases were confirmed by pathological diagnoses to have simple hyperplasia.1 case was of atypical hyperplasia,and 3 cases were of endermetrial cancer.Of the 17 cases between 50-59,5 cases had normal uteri and 6 cases had hyperplasia,11 cases had homogeneous hyperplasia and 6 cases had heterogeneous hyperplasia.Pathological diagnoses confirmed 13 cases of simple hyperplasia,2 cases of tumor-like hyperplasia and 2 cases of endometrial adenocarcinoma.Of the 2 cases above 60,1 cases had normal uterum,another one had hyperplasia,and both of endometrial thickening and heterogeneous echo.The Pathological diagnoses confirmed 1 cases of simple hyperaplasia and another cases of glandular sac-like hyperplasia.31 cases（55.36%） had their unilateral ovaria reach the cyst-like dark area,with the diameter being shorter than 3.5cm,and 8 cases（14.29%） had their bilateral ovaria reach the cyst-like dark area,with the diameter being shorter than 2.9cm.Conclusion B-ultrasonic imaging can accurately show the changes in myometria and endometria and can be effectively used to diagnose Swiss cheese endometrial hyperplasia.
