摘要
目的比较微型钢板内固定与桡骨小头切除术治疗MasonIII、IV型桡骨小头粉碎性骨折的临床疗效。方法2004年9月~2007年8月期间,笔者手术治疗32例成人桡骨小头粉碎性骨折病例,其中MasonIII型26例,MasonIV型6例,根据临床具体情况随机分为2组,A组17例采用切开复位微型钢板内固定,B组15例采用桡骨小头切除术,术后定期随访,根据患者术后肘关节活动、疼痛、稳定性及肌力情况依照Broberg和Morrey的肘关节评分标准进行功能评价。结果32例患者随访10~24个月,平均15个月。A组优9例,良7例,可1例;B组优4例,良7例,可3例,差1例;A、B两组优良率分别为94.1%和73.3%,A组疗效明显优于B组。结论与桡骨小头切除相比,切开复位微型钢板内固定治疗MasonIII、IV型桡骨小头粉碎性骨折可获得较好的肘关节功能恢复。
Objective To compare the clinical results of minor plate internal fixation and radial head resection in the treatment of Mason type-Ⅲ, Ⅳ radial head fracture. Methods During 2004 September to 2006 August ,32 adult patients were enrolled in this study(17 patients, group A ,were treated by minor plate internal fixation and 15 patients,group B, were treated by radial head resection),following up the cases regularity. Broberg and Morrey criteria were used to analyze the elbow function on the basis of motion ,pain,stability and strength. Result 32 cases were followed up for a mean time of 15 months, the therapeatic effect were evaluated as excellent 9, good 7, fair 1 in group A and 4,7,3,1 respectively in group B.The therapeutic effect in group A was significant superior to that in group B. Conclusion Compared with radial head resection,minor plate internal fixation can result in better elbow function recovery in the treatment of Mason type-Ⅲ, Ⅳ comminuted fracture of radial head.
