摘要
目的探讨后适应联合重组人促红细胞生成素(EPO)对大鼠心肌缺血再灌注损伤的保护作用及其可能机制。方珐将40只大鼠结扎左冠状动脉前降支后随机分为4组,缺血再灌注组、后适应组、EPO组及EPO+后适应处理组(联合组),每组10只。测定梗死区与左心室重量之比、血清肌钙蛋白浓度、血流动力学参数,评价后适应和EPO处理对心脏结构和功能的影响。采用ELISA及Western blot方法观察各组Akt和内皮型一氧化氮合酶(eNOS)的变化。结果与缺血再灌注组比较,后适应组、EPO组、联合组均可缩小心肌梗死面积,降低血清肌钙蛋白浓度,改善心功能(Pd0.05),其中联合组较后适应组作用加强。后适应组和EPO组可促进Akt和eNOS的磷酸化(P〈0.05),联合组对Akt通路磷酸化有明显的促进作用。结论后适应联合EPO可对大鼠缺血再灌注损伤心脏提供进一步的保护作用,其机制可能是通过磷脂酰肌醇三羟基激酶通路发挥作用。
Objective To explore the protective effect of recombinant human erythropoietin (EPO) and post-conditioning(post-con) against rat myocardial ischemia-reperfusion injury and its possible mechanism. Methods Forty open-chest rats endured coronary occlusion for 30 minutes fol- lowed by reperfusion for 3 hours. They were divided into 4 groups: control, post-con, EPO and post-con+EPO (n =10 for each group). The hemodynamic parameters,infarct size of hearts and plasma troponin level were observed in all rats. The phosphorylation states of Akt and endothelial nitric oxide synthase(eNOS) were determined by ELISA and Western blot respectively. Results Compared with control group, infarct size and plasma troponin level were reduced and hemodynamie parameters were improved in post-con,EPO and post-con+EPO groups (P 〈0.05), especially in post-con+EPO group (P 〈0.05). ELISA and Western blot analysis demonstrated that a significant increase in phosphorylation of Akt and eNOS were induced in all intervention groups, especially in the post-con+EPO group (P 〈0.05). Conclusion Post-conditioning combined with erythropoietin can provide further cardioprotection against the rat heart ischemia-reperfusion inju- ry by activating phosphatidylinositol 3 kinase-Akt pathway.
