产CTX-M型超广谱β-内酰胺酶大肠埃希菌基因型分析 被引量:1

摘要 目的了解我院临床分离大肠埃希菌产CTX-M型超广谱β-内酰胺酶(ESBLs)的发生率和基因型分布情况。方法确证试验鉴别临床分离大肠埃希菌中产ESBLs株,PCR扩增CTX-M耐药基因,PCR-RFLP和DNA测序确定CTX-M基因型。结果确证实验证明,88株大肠埃希菌中有57株ESBLs阳性,其中54株CTX-M基因扩增阳性。PCR-RFLP分析发现,10株携带CTX-M-1组基因,39株携带CTX-M-9组基因,另外5株同时携带CTX-M-1和CTX-M-9组基因。测序证实7株CTX-M-9组扩增产物均为CTX-M-14型,3株CTX-M-1组扩增产物均为CTX-M-3型。结论大肠埃希菌中产CTX-M型ESBLs的检出率较高,其基因型以CTX-M-14和CTX-M-3型为主。 Objective To study the positive rate of CTX-M-prnducing extended -spectrum β-laetamases(ESBLs)in clinical isolates of Escherichia coli, and to investigate its genotype. Methods ESBLs-producing Escheriehia coli of clinical isolates were confirmed by confirmatory test. CTX-M gene was amplified by PCR. PGR-RFLP and DNA sequencing were applied to confirm its genotype. Results Confirmatory test showed that 57 strains were identified as ESBLs positive in 88 clinical isolates. CTX-M gene was amplitfied in 54 strains by PCR.The results of PCR-RFLP showed there were 10 isolates with CTX-M-1,39 isolates with CTX-M-9,5 isolates with CTX- M-1 and CTX-M-9 simultaneously.7 isolates with CTX-M-9 and 3 isolates with CTX-M-1 were identified as CTX-M-14 and CTX-M-3 genotype by DNA sequencing. Conclusion The precalence of CTX-M ESBLs among clinical isolates of Escherichia coli is high, and the dominating genotypes are CTX-M-14 and CTX-M-3.
