摘要
近年来胃电刺激备受关注,其原由为电子技术发展迅猛、功能性胃肠疾病缺乏有效治疗手段及胃电刺激对胃轻瘫和肥胖患者治疗巨大进展。本文系统的回顾了胃电刺激对肥胖患者治疗的作用机制。本综述通过对Medline检索,其结构如下:①胃电刺激方法;②胃电刺激肥胖治疗的临床运用;③胃电刺激作用机制;④胃电刺激前景展望。目前研究显示胃电刺激能有效地减少食物摄入和体质量,是因其可抑制胃动力、激活饱食神经元并改变中枢和外周饱食和食欲相关性肽和激素的作用。然而,为进一步证实胃电刺激的临床有效性,需开发新一代的胃电刺激器和进行临床对照试验。
Recently gastric electrical stimulation (GES)has received increasing attention among researchers and clinicians, attributed to the advancement of electronics, the lack of effective therapies for gastrointestinal functional diseases and the applications of GES in patients with gastroparesis and obesity. The aim of this article was to give a systematic review on the mechanisms gastric electrical stimulation applied for the treatment of obesity. Based on the general search of Medline, the review was structured as follows:(1) Methods of gastric electrical stimulation ; (2) clinical applications of GES for obesity; (3)Mechanisms of gastric electrical stimulation ; (4) future development of gastric electrical stimulation. GES has been shown to be effective in reducing food intake and body weight via its inhibitory effects on gastric motility,the activation of satiety neurons and alterations in central and peripheral peptides associated with satiety and appetite. However,development of new device is needed and controlled clinical studies are required to further prove clinical efficacy of GES for obesity.
