不同 Ca^(2+)浓度的 St . Thomas 停搏液对未成熟心肌的保护作用

EFFECT OF ST.THMOMAS CARDIOPLEGIA AT DIFFERENT CALCIUM CONCENTRATIONS ON IMMATURE MYOCARDIUM

摘要 目的研究含不同Ca2+浓度的St.Thomas停搏液对未成熟心肌的保护作用。方法采用20℃低温90min缺血再灌注幼兔离体心脏灌注模型。结果再灌注10min,高Ca2+组LVDP、±dp/dt恢复率明显高于低Ca2+及中Ca2+组(P<0.05,P<0.01)。但再灌注20min后,高Ca2+组左室功能呈下降趋势,再灌注末,与其它组间无显著差异,与之相比,再灌注期间,低Ca2+及中Ca2+组左室功能则呈渐进性恢复。结论再灌注后,高Ca2+组心肌细胞外间隙滞留的Ca2+进入细胞内,加速了Ca2+诱导的ATP耗能过程,高Ca2+浓度的St.Thomas停搏液对未成熟心肌不能提供满意的保护作用。 Objective To determine the myocardial protective effect of St.Thomas cardioplegia at different calcium concentrations on immature myocardium.Methods Isolated perfused neonatal rabbit hearts were subjected to 20℃ hypothermic 90 minutes of global ischemia in Langendorff mode of operation.Results Although the recovery of LVDP ±dp/dt max at calcium content of 2.4 mmol/L was significantly higher than that at 0.6 and 1.2 mmol/L calcium (P<0.05,P<0.01,respectively) after 10 minutes of reperfusion, the hemodynamic recovery was found declining in the next 20 minutes. By the end of 30-min reperfusion, the functional recovery at 2.4 mmol/L calcium did not differ from that at 0.6 and 1.2 mmol/L calcium. Conversely, postischemic LV function at 0.6 and 1.2 mmol/L calcium got improved gradually during the 30 minutes of reperfusion.Conclusion Calcium accumulated in the extracellular space during ischemia enters myocardial cell after reperfusion, and accelerates the ATP consumption induced by calcium, which would be possibly responsible for the results noted in this study. It is concluded that St.Thomas cardioplegia at high calcium concentration can not provide immature myocardium with optimal myocardial protection.
