经食管实时三维超声心动图评价牛心包主动脉瓣成形术1例 被引量：1

摘要 患者男，35岁。以“劳力性胸痛、胸闷、心慌、气喘2月”入院。查体：心尖搏动向左移位，无震颤，心界向左扩大，心率78次／min，律齐，肺动脉瓣第二音无亢进，主动脉瓣第二音〉肺动脉瓣第二音，主动脉瓣第二听诊区闻及舒张期叹气样杂音，传导广泛。经胸二维超声心动图检查：左房扩大（前后径4．5cm），左室明显扩大（前后径8．5cm）。主动脉右冠瓣增厚，回声增强，舒张期右冠瓣越过瓣环水平脱向左室流出道，与余瓣对合错位，关闭不良，其左室流出道可见重度偏心性反流信号，反流束沿二尖瓣前瓣冲击至左室心尖部。诊断：主动脉右冠瓣脱垂并重度关闭不全。
