摘要
目的探讨妊娠期妇女葡萄糖耐量筛查意义及其糖代谢异常的相关因素。方法对1608例孕24—28周的孕妇进行50g葡萄糖耐量筛查,1小时血糖≥7.8mmol/L者再行75g葡萄糖耐量试验。结果妊娠期葡萄糖耐量筛查阳性率为19.80%,糖代谢异常检出率为7.46%,其中妊娠期糖尿痛的检出率为2.49%,妊娠期葡萄糖耐量减低发生率为4.98%;糖代谢异常者的平均年龄大于糖代谢正常者(t=6.09,P〈0.05),同时年龄≥35岁糖代谢异常孕妇明显多于〈35岁者(χ^2=23.26,P〈0.05);孕前身体质量指数≥25的孕妇发生糖代谢异常高于孕前身体质量指数〈25者(χ^2=26.08,P〈0.05);产次多的孕妇容易发生糖代谢异常,产次≥2的孕妇发生糖代谢异常高于0产次、1产次者(χ^2分别为31.60、10.00,均P〈0.017)。妊娠期糖代谢异常的高危因素有高龄、肥胖和多产次。结论对妊娠期24—28周孕妇进行50g葡萄糖耐量筛查是必要的,对伴有高危因素的孕妇应重点监测。
Objective To investigate significance of glucose tolerance screening among pregnant women and related factors of abnormal glucose metabolism. Methods 1 608 pregnant women at 24-28 weeks of gestation were sereened for glucose tolerance with 50g of glucose. The pregnant women whose blood glucose level was≥7.8mmol/L at 1 hour after oral administration of glucose received 75g of oral glucose tolerance test again. Results The positive rate of glucose tolerance screening among pregnant women was 19.8% and the detectable rate of abnormal glueose metabolism was 7.46%. Of which, the positive detectable rate of gestational diabetes mellitus was 2.49%, and the rate of gestational decreased glucose tolerance was 4.98%. The average age of pregnant women with abnormal glucose metabolism was older than that of those with normal glucose metabolism( t = 6.09, P 〈 0.05 ). The number of pregnant women with abnormal glucose metabolism aged ≥35 years old was significantly more than number of those pregnant women with abnormal glucose metabolism aged ≤ 35 years old (χ^2 = 23.26, P 〈 0.05 ). The rate of abnormal glueose metabolism among the pregnant women with pregestafional BMI ≥ 25 was higher than that among those pregnant women with pregestafional BMI ≤ 25 (χ^2 = 26.08, P 〈 0.05 ). The pregnant women with more parity were susceptible to develop abnormal glueose metabolism: the rate of abnormal glucose metabolism among the pregnant women with ≥2 parity was significantly higher than that among those pregnant women with 0 parity or 1 parity (χ^2 = 31. 60, 10. 00, respectively, both P 〈0. 017 ). The risk factors of gestational glucose metabolism included older age of pregnant women, obesity, and more parity. Condusion It is necessary to screen glucose tolerance among those pregnant women at in 24 - 28 weeks of gestation. Those high risk pregnant women should be monitored intensively.
