摘要
目的研究肾小管损伤标志物在儿科疾病中的应用价值,并比较其在疾病中联合应用的价值和特异性。方法采用酶联免疫吸附法(ELISA法)和比色法,测定2007年2月至2007年10月间多病因导致的肾损伤性疾病患者尿中β2微球蛋白(β2-MG)、视黄醇结合蛋白(RBP)、N-乙酰-β-D氨基酸核苷酶(NAG)I、gG、微白蛋m-ALB的水平,共142例,年龄范围6个月-17岁,分为肾病组、糖尿病组、过敏性紫癜组3组,并与41例健康儿童对照组进行比较。应用SPSS13.0统计软件分析。结果肾病组尿中标志物水平均显著高于正常对照组P<0.05,且联合应用100%为阳性结果。所测标志物水平比其它各组升高明显,差异有显著性意义(P<0.05);尿中β2-MG、RBP、NAG在糖尿病组(Ⅰ,Ⅱ型)中测定水平增高较高,与其它各组有显著差异(P<0.05);尿中IgG、m-ALB在过敏性紫癜组中测定水平较高,其中IgG水平在过敏性紫癜组和糖尿病组中均增高且差异无显著性(P>0.05),而m-ALB与其它各组有显著性差异(P<0.05)。结论当多种原因造成肾脏损伤,根据病因、症状体征及肾损伤程度等的不同,可测得多种肾损伤标志物的变化,辅助临床早期发现肾损伤部位、程度,指导临床医生选择正确治疗方案,合理用药及判断预后。
Objective To research on the value of markers on renal tubular injury in pediatric disease,and compare the value and specificity of the joint application on these markers in different diseases. Methods To use Enzyme-hnked immunosorbent assay （ELISA） and colorimetry,from February 2007 to October 2007,the level of β2-MG, RBP, NAG,IgG,m-ALB in the urine of patients with renal injury by variety of causes can be detected, and compared with the group of 41 healthy, control children group, applying SPSS13.0 statistical analysis software to analyze. Results The level of markers in urine of the nephrotic group were significantly higher than the normal control group P 〈 0.05,the joint application results are 100% positive. The measured levels of markers were significantly increased signifificantly （P 〈 0.05） titan the other groups; in the diabetic group （ Ⅰ , Ⅱ type）, the determination of β2- MG, RBP, NAG in urine showed a higher level increased, there were significantly different between other groups （ P 〈 0.05） ; there were higher determination level of urinary IgG, m - ALB in anaphylactoid purpura group, the level of IgG in group of anaphylactoid purpura and diabetes both increased and had no significant difference （ P 〉 0. 05）, and the level of m-ALB was significant difference between other groups （ P 〈 0. 05）. Conclusion When kidney were injured by multiple causes, according to the different degree of causes, symptoms and signs of kidney damage, and so on, many changes on markers of renal injury can be measured, it can be assisted on detecting the site and degree of early clinical renal injury,it can guide clinicians to choose the correct treatment programs,use drug reasonably, judge and prognosis accurately.
