心脏直视手术省血技术综合应用 被引量:2

摘要 目的探讨省血技术在心脏直视手术中的综合应用,以期在心内直视手术围手术期少输血或不输血。方法选择2000年7月~2003年2月心脏瓣膜直视手术的回顾性病例80例为A组(对照组1),选择2004年3月~2007年6月瓣膜直视手术病例160例,随机分两组,单纯精确手术操作、仔细止血、加用或不用止血药病例80例为B组(对照组2),省血技术综合应用病例80例为C组(实验组)。监测3组病人围手术期输血量,术后引流量,术前及出院前1dHCT及相关临床记录(包括体外循环灌注温度,血流动力学稳定情况,电解质,pH,凝血分析,术后病人转归等)。结果A组:围手术期平均用血量812±196ml,术后24h引流量622±124ml,引流总量822±106ml;B组:围手术期平均用血量410±113ml,术后24h引流量305±138ml,引流总量398±126ml;C组:围手术期平均用血量376±105ml,术后24h引流量200ml以下,引流总量346±108ml。C组和B组围手术期输血总量、24h引流及引流总量少于A组(P<0.01);C组围手术期输血量、24h引流及引流总量少于B组(P<0.05)。结论省血技术综合应用能减少心内直视手术输血量。
