摘要
为了解血管内皮细胞生长因子对皮瓣成活率的影响，选用３０只ＳＤ大鼠，背部７．５ｃｍ×３ｃｍ随意皮瓣的动物模型。术中和术后２４小时局部注射血管内皮细胞生长因子（ＶＥＧＦ）４０ｎｇ／瓣，以肝素１６Ｕ／瓣和生理盐水８００μｌ／瓣作为对照组。采用常规组织学和组织形态计量学方法，对皮瓣成活率和皮下血管断面密度进行检测，观察其对皮瓣成活率的影响。结果表明，术后３天皮瓣成活率：ＶＥＧＦ组为（８５．８５±２．２０６）％，肝素组为（８２．５７±１．５５１）％，生理盐水组为（８２．６２±１．５７６）％，各组间差异无显著性（Ｐ＞０．０５）；术后１１天皮瓣成活率：ＶＥＧＦ组为（８３．７９±２．４８４）％，肝素组为（７３．１５±２．５００）％，生理盐水组（７３．０６±２．０１８）％，ＶＥＧＦ组与其它两组间差异有显著性（Ｐ＜０．０５）。对皮下血管断面的组织形态计量学检查发现，ＶＥＧＦ组皮下血管断面密度在皮瓣的中、远端与肝素组和生理盐水组差异有显著性（Ｐ＜０．０５和Ｐ＜０．０２）。认为，血管内皮细胞生长因子的局部应用，通过加速皮瓣内血管再生，促进皮瓣与受区间血循环的重建，从而改善了大鼠超比例皮瓣远端的血供状态，对提高皮瓣成活率有一定的意义?
In order to study the effect of vascular endothelial cell growth factor (VEGF) on the survival of skin flap 30 SD rats were used. A randomized flap measuring 7.5 cm×3.0 cm was created on the back of each SD rat. The treatment group (n=10) received VEGF 40 ng/flap by subcutaneous injection with microinjector during and 24 hours after operation. The control groups received heparin 16 U/flap (n=10) or normal saline 800 μl/flap (n=10). After operation, on the 3rd and 11th day, the survival rate of the skin flaps and the dermovascular density of each flap were investigated by histological and histo morphometrical examination. The results showed that there was no significant difference in the survival rate between the treatment group and the controls on the 3rd day after operation, while on the 11th day, there was a significant difference between them, and the survival rate was much higher in the treatment group. Besides, dermovascular density was much more increased in the treatment group than that in the controls, especially in the distal 1/3 of the flap (P<0.02). The conclusion was that VEGF could accelerate the vascularization of the flap and enhance the survival rate of the flap.
