摘要
目的探讨在血细胞分离过程中,葡萄糖酸钙用药的最佳时机和方式。方法将216例行血细胞分离患者随机分成两组,观察组108例在分离血细胞开始前采取匀速、低浓度补钙法,即将10%葡萄糖酸钙10 ml用生理盐水40 ml稀释成50 ml溶液,用4号针头静脉注射;对照组108例,采用随机补钙法,给药时机主要在血细胞分离进行中,抽取10%葡萄糖酸钙10 ml,用8号针头缓慢静脉注射。两组静脉注射时间均为6 min,观察、询问并记录患者的感受与不适。结果观察组发生不良反应4例次,即头痛头晕、恶心呕吐、胸闷心慌、口周麻木各1例次,对照组发生不良反应共53例次,分别为头痛头晕10例次、恶心呕吐4例次、胸闷心慌13例次、喉咙及全身发热16例次,口周麻木10例次,两组间差异有统计学意义(P<0.01)。结论在细胞分离前采取匀速、低浓度静脉给药方法补钙,能减少各种不良反应的发生,提高患者在分离过程中安全性和舒适度。
Objective To explore the optimal time and method of intravenous administration of calcium gluconate during blood cell separation. Methods A total of 216 tumor patients undergone cell separation were evenly divided into two groups at random. Every patient in group A,before blood cell separation, was intravenously administrated with 50 ml calcium gluconate solution (10 ml of 10% calcium gluconate diluted with 40 ml of normal saline) through 50 ml injection syringe with 4 size needle, injected for 6 min. In group B (contrast group), 10 ml of 10% calcium gluconate were given randomly in blood cell separation through 10 ml injection syringe with 8 size needle, slowly injected for equal time. Patients' feelings and discomforts were observed, asked and recorded. Results Adverse effects in the group A appeared 4 times, with headache, nausea and vomiting, chest tightness and palpitation, and perioral numbness 1 time repetitively; while in group B, adverse effects appeared up to 53 times, with 10 headache and dizziness, 4 nausea and vomiting, 13 chest tightness and palpitation, 16 fever in the throat or overall the body, and 10 perioral numbness. The differences of adverse effects were of statistical significance between the two groups (P〈0.01). Conclusion Intravenous administration of calcium gluconate of a low concentration evenly distributed in the initial stage can reduce adverse effects, and enhance the safety and comfort of the patients in blood cell concentration.
