摘要
目的探讨乳突根治术失败原因及预防措施,以提高手术技巧及干耳率。方法对我科2000～2007年收治的37例乳突根治术失败患者的再次乳突手术所见进行分析,并行二次(或三、四次)乳突根治术。结果术后1～3个月患者原有症状消失、干耳。结论乳突根治术失败原因主要是术者对中耳乳突解剖不熟、基本功不过硬、临床经验不足导致病灶残留、引流不畅和乳突尖开放不够及某些特殊情况所致。术中彻底去除病灶,保证术后引流通畅及特殊情况的处理,是防止二次手术的关键。
Objective To discuss the causes of radical mastoidectomy failure and the prevention. Methodss An analysis was made to what was observed in 37 re - operated cases after the failure of the first operation .and a second （or third and even fourth） radical mastoidectomy was performed. Results The previous symptoms disappeared 1 to 3 months after operation and dry ears were gained. Conclusions Lesion residue, obstructed drainage and inadequate opening of mastoid tip caused by limited understanding of the anatomy ofmiddle ear and mastoid, poor skill and lack of clinical experience are the main reasons for the failures of radical mastoidectomy; complete removal of the lesions in operation and unobstructed postoperative drainage are the key point to avoid a second operation.
