摘要
目的检测环氧化酶COX-2及凋亡抑制基因Bcl-2在肾癌组织中的表达,探讨评估肾细胞癌预后因素的指标。方法应用免疫组化技术检测40例肾细胞癌标本中COX-2、Bcl-2的表达,分析两者的关系并探讨其与肾细胞癌的临床分期、病理分级、病理类型及浸润、转移的关系。结果COX-2和Bcl-2在肾癌中的表达率分别为25/40和29/40。主要为癌组织的表达,而在癌旁正常组织中无表达或弱表达。COX-2的高表达与肿瘤的分期(P<0.01)、分级(P<0.01)与分型(χ2=10.761,P<0.01)差异有统计学意义。Bcl-2的高表达与肾癌的分期(P<0.01)、分级(P<0.01)差异有统计学意义,与肾癌的细胞类型的差异无统计学意义。COX-2和Bcl-2的表达呈正相关(P<0.01)。结论肾癌组织中COX-2和Bcl-2的高表达与肾癌的分期、分级及肿瘤侵袭、转移有关,提示二者共同高表达有助于肾癌的诊断,可作为判断肾癌预后不良的依据。
Objective To detect the expression of cyclooxygense (COX-2)and Bcl-2 in renal cell carcinomas and study the prognostic predictor for renal cell carnoma. Methods The expression of COX-2 and Bcl-2 in 40 primary RCC specimens was examined by immunohistochemical procedures. To evaluate the relationship between COX-2 and Bcl-2, and study their relationship to chnical staging, pathologic grading, history type, invasion and metastasis in renal cell carcinoma. Results The positive rate of COX-2 and Bcl-2 in renal cell carcinoma was 25/40 and 29/40 respectively. But there was no or weak expression in non-cancerous tissues of kidney. Overexpression of COX-2 in renal cell carcinomas was related to its clinical staging, pathologic grading and history types. Overexpression of Bcl-2 in renal cell carcinomas was related to clinical staging, pathologic grading but no related to its history types. The expression of COX-2 was positively correlated with the expression of Bcl-2. Conclusion The high level expression of COX-2 and Bcl-2 in renal cell carcinoma may play a crucial role in its clinical staging, pathologic grading, invasion and metastasis in renal cell carcinoma. Indicating that COX-2 and Bcl-2 in high level might be helping to the diagnosis and also an important indicator in predicting the bad prognosis.
