摘要
目的:观察采用中药及早期功能锻炼对手法复位经皮交叉克氏针及合页石膏托固定微创治疗儿童肱骨髁上骨折的临床疗效。方法:65例肱骨髁上骨折患儿,在非牵引状态下纠正侧方移位及旋转移位,术者双拇指按压肱骨髁上骨折部位,双手余四指环抱肱骨中段背侧,使骨折部复位,骨折部向外推移少许并外倾5°￣8°后经皮交叉克氏针固定,均屈曲90°旋前位石膏托固定。术后3 d,部分折断肘部石膏托,逐渐行屈肘被动功能锻炼,同时结合中药外用治疗。结果:65例患儿中,优37例,良20例,可6例,差2例。57例肘关节功能恢复正常,8例遗留肘内翻,无骨髓炎发生。合并有10例神经损伤的患儿,均在短期内完全恢复感觉和功能。结论:应用手法复位经皮交叉克氏针固定,同时结合早期功能煅炼与中药外用治疗儿童肱骨髁上骨折可获得满意疗效,避免了手术对肘部造成的损伤,并在较短时间内使肘关节功能恢复正常。
Objective：To observe the clinical therapeutic effect of Chinese herbs and early function exercise on supracondylar humeral fracture treated with minimal therapy of percutaneous Kirschner wire in manipulation replacement and hinge gypsum fixation in children. Methods：Lateral displacement and rotational displacement were rectified in 65 cases of supracondylar humeral fracture at non-traction state. The operator pressed the fractured region of supracondyle of humerus with both thumbs and held the dorsal side of the middle section of humerus with the rest figures of both hands to have the fractured region replaced. After the fractured region was shifted slightly outward and laterally by 5^o- 8^o,it was fixed with percutaneous Kirschner wire,in following,the fixation with gypsum holder was applied in elbow flexion 90^o with the forearm in pronation. On the 3rd day after operation,the gypsum holder in elbow region was broken off partially,and the pas- sive function exercise of elbow flexion was underway, simultaneously, Chinese herbal medicine was applied externally. Results：Of 65 cases, 37 cases were manifested as being excellent, 20 cases as good,6 cases as fair and 2 cases as poor. 57 cases presented normal recovery of elbow function, 8 cases was of cubitus varus left,but without osteomyelitis occurred. 10 cases complicated with nerve injury had all recovered their sensitivity and motor function completely in a short term. Conclusion：Satisfactory therapeutic effects have been achieved in supracondylar humeral fracture in children treated with percutaneous Kirschner wire and in combination with early function exercise and external application of Chinese herbs,which avoids the injury of operation to the elbow and promotes the motor function recovery to the normal in a quite short time.
