摘要
目的探讨阻塞性睡眠呼吸暂停综合征（OSAS）与高血压的关系，为此种类型高血压的治疗提供依据。方法选取合并有OSAS的高血压患者52例作为实验组（OSAS组），同期住院单纯高血压48例作为对照组，同步进行24h动态血压及多导睡眠仪检测；两组患者均常规低盐饮食、口服药物治疗2周，比较治疗前后24h动态血压；OSAS组中的18例降压效果差的患者同时接受气道正压通气（CPAP）治疗2周，观察血压控制情况。结果两组之间的白天平均收缩压（DSBP）、白天平均舒张压（DDBP）、夜间平均舒张压（NDBP）差异无统计学意义，OSAS组的夜间平均收缩压（NSBP）明显高于对照组（P〈0．05）；药物降压两周后，对照组血压较OSAS组明显下降（P〈0．05），OSAS组药物治疗前后无明显变化；18例药物治疗效果差的合并有OSAS的患者经CPAP治疗2周后血压显著下降。结论OSAS是引起高血压的原因之一，单纯药物治疗对与OSAS有关的高血压患者疗效欠佳，CPAP治疗有助于此类患者血压的控制。
Objective To investigate the relationship between hypertension and obstructive sleep apnea syndrome, provide some reference in clinical. Methods Fifty-two patients with hypertension and obstructive sleep apnea syndrome（OSAS） as experimental group,48 cases of single hypertension as control group, all patients were monitored simultaneously by PSG and ABPM; Two groups conducted low salt diet and medicine for two weeks, compared blood pressure before treatment and after treatment; 18 patients .received poor results in decrease pressure of experimental group going on CPAP the same time two weeks, observed the changes of blood pressure. Results There was no significant difference in the mean DSBP, DDBP, NDBP between the two groups, Compared with control group, the mean NSBP was higher remarkably; After the first two weeks treatment, the pressure level of control group was decreased remarkably （P〈0.05）, that of experimental group was not defer from before and after treatment; The blood pressure of 18 patients that didn＇t get good control at first step of treatment was decreased remarkably after the second two weeks of CPAP treatment. Conclusion OSAS may be considered as one of the causes of hypertension, CPAP should be applied if patients with this special kind of hypertension could not get good control only use medicine.
