摘要 背景:课题设计研发的颊侧多曲簧矫治器是一种新型的可全天戴用的矫治器,主要用于乳、替牙期AngMII类错[牙合]畸形,具有多曲簧柔和的弹力效应,在推上颌骨向前促进其发育的同时,可抑制下颌的过度向前发育。目的:应用颊侧多曲簧活动矫治器柔和的生物力学特点,治疗早期Angle Ⅲ类错[牙合]的病例,分析部分硬组织测量值的前后变化。设计、时间及地点:自身前后对比观察,于2006-01/2008—04在哈尔滨医科大学附属第二医院口腔正畸科完成。对象:选择功能性或骨性下颌前突的安氏Ⅲ类错[牙合]患者31例:女15例,男16例;年龄5~13岁。矫治器:自行研发的颊侧多曲簧活动矫治器。方法:对31例早期Angle Ⅲ类错[牙合]患者采用颊侧多曲簧活动矫治器进行颌间交互支抗固位。矫治前后拍摄X射线头颅侧位定位片。主要观察指标:患者矫治前后头影测量反映部分硬组织之间关系的角变化。结果:矫治前后头影硬组织间关系的测量表明:上颌位置前移,上颌长度增加。下颌位置向下后方向生长。上下颌骨侧面突度趋于正常,骨性Ⅲ类面型得到改善。上前牙前移,下切牙长轴舌向倾斜。结论:颊侧多曲簧活动矫治器柔和的弹力作用,在促进上颌向前生长发育的同时,又抑制了下颌的过度生长发育,并使下颁功能性的后退,从而解除反牙台。 BACKGROUND: Buccal multiloop removable appliance is a novel appliance which can be worn for a whole day. It is primarily used to treat Angle Ⅲ malocclusion at milk teeth and mixed dentition periods. In addition, it can promote maxilla development and inhibit mandible excessively forward development. OBJECTIVE: To investigate hard tissue changes before and after the treatment by buccal multiloop removable appliance in early stage of class Ⅲ malocclusion. DESIGN, TIME AND SETTING: Self-contrast observation was performed at the Department of Orthodontics, Second Hospital of Harbin Medical University between January 2006 and April 2008. PARTICIPANTS: Thirty-one patients with early stage of class Ⅲ functional or bony malocclusion were selected, including 15 females and 16 males aged 5 13 years. The appliance was self developed. METHODS: Buccal multiloop removable appliance was used to treat early stage of class Ⅲ malocclusion in 13 patients. Cranial lateral X-ray was taken before and after treatment. MAIN OUTCOME MEASURES: Angle changes of some hard tissues before and after treatment. RESULTS: The cephalometric analysis before and after correction showed that upper mandible shifted forward, and upper mandible length was increased; mandible shifted downward and posterior. Upper and lower mandible profile convexity was normal, and class Ⅲ malocclusion was improved. In addition, the upper and anterior teeth shifted forward, and lower long axis of the incisors sloped to the tongue. CONCLUSION: The buccal multiloop removable appliance can promote maxilla forward growth, restrain excessive development of mandible, and keep it back functionally to eliminate crossbite occlusion.
